降糖通脉片联合胰岛素注射治疗对2型糖尿病患者凝血状态、微血管并发症的影响

摘　　要：目的:研究降糖通脉片联合胰岛素注射治疗对2型糖尿病患者凝血状态及微血管并发症的影响。方法:采取随机数字表法将2015年4月至2016年9月我院收治的84例2型糖尿病患者分为两组,各42例。对照组单纯予以胰岛素注射治疗,观察组采取降糖通脉片联合胰岛素注射治疗方案,比较2组治疗前后血糖浓度,球后动脉舒张末期最小流速(EDV)、收缩期最大流速(PSV)、血流阻力指数(RI),D二聚体(D-D)、纤维蛋白原(FIB)水平、血小板聚集率(MPAR),血浆内皮素水平、微血管并发症情况。结果:经过治疗,观察组FPG、2hP G、D-D、FIB、血浆内皮素水平、MPAR均显著低于对照组,且EDV、PSV、RI明显高于对照组;观察组微血管并发症总发生率为4.76%,显著低于对照组19.05%。结论:对2型糖尿病给予降糖通脉片联合胰岛素注射治疗,可有效改善血糖浓度与球后血管血流动力学指标,优化凝血状态,降低微血管并发症风险。Objective： To study the effect of Jiangtangtongmai Tablets combined with insulin injection on coagulation status and microvascular complications in patients with type 2 diabetes mellitus. Methods： A total of 84 patients with 2 diabetes mellitus treated in our hospital from April2015 to September 2016 were selected and divided into two groups by random number table method,with 42 cases in each group. Patients in the control group were treated with insulin injection alone,and those in the observation group were treated with Jiangtangtongmai tablets combined with insulin injection. The blood glucose level,the arterial minimum end diastolic velocity（ EDV）,the peak systolic velocity（ PSV）,resistance index（ RI）,D dimer（ D-D）,fibrinogen（ FIB） level,the platelet aggregation rate（ MPAR）,the plasma endothelin level and microvascular complications were compared between two groups. Results： After the treatment,levels of FPG,2 h PG,D-D,FIB,endothelin and MPAR in the observation group were significantly lower than those in the control group（ P〈0. 05）,and levels of EDV,PSV and RI were significantly higher than those in the control group（ P〈0. 05）. The total incidence of microvascular complications in the observation group was significantly lower than that in the control group（ 4. 76% vs 19. 05%）（ P〈0. 05）. Conclusion： Jiangtangtongmai tablets combined with insulin injection can effectively improve the blood glucose level and hemodynamic indexes of patients with 2 diabetes mellitus,optimize the coagulation status and reduce the risk of microvascular complications.
