维持性血液透析患者认知功能障碍及相关因素分析

摘　　要：目的评估维持性血液透析患者认知功能障碍发生率及探讨影响认知功能的因素。方法选择2016年5月至2017年4月某院维持性血液透析患者110例,采用中文版蒙特利尔认知评估量表行认知功能评分,收集人口统计学资料及透前临床化验指标。根据认知功能评分,将患者分为CI组与非CI组,比较2组患者的临床及实验室指标。采用回归分析影响维持性血液透析患者认知功能的因素。结果 95例患者发生CI,发生率为86.4%。CI组与非CI组在年龄、C反应蛋白、白蛋白及血尿酸水平均有明显差异(P<0.05)。多元线性回归分析显示:年龄、受教育年限、血尿酸水平是影响维持性血液透析患者认真功能的因素(P<0.05)。结论维持性血液透析患者认知功能障碍的患病率高,年龄、受教育年限、血尿酸水平与认知功能相关。Objective To explore the prevalence of cognitive impairment and its corresponding factors in hemodialysis patients. Methods This study selected 110 patients who underwent hemodialysis in one hospital, and cognitive performance was measured with Chinese version of The Montreal Cognitive Assessment（Mo CA）. The reseacher registered demographic data and recorded biochemical characteristics. The patients were divided into CI group and CI-free group, according Mo CA score. The clinical and laboratory data were compared between the two groups. Linear-regression analysis was performed to evaluated the factors that effected cognitive function of hemodialysis patients. Results 95 hemodialysis patients were classified with cognitive impairment and the prevalence of cognitive impairment reached 86.4%. The age, c-reactive proten,albumin, and serum uric acid level were significantly different between CI group and CI-free group（P0.05）. Multi-factor linear-regression analysis indicated that age, years of education, and serum uric acid level were independent factors that effected cognitive function in hemodialysis patients. Conclusion The incidence of CI in hemodialysis patients is high, and age,years of education, and serum uric acid level were relevant with cognitive function in hemodialysis patients.
