清毒调血方治疗脓毒症疗效观察及对炎症因子的影响

摘　　要：目的观察在常规西医治疗的基础上加用清毒调血方口服或鼻饲治疗脓毒症的临床疗效及对炎症因子、HMGB1的影响。方法将70例脓毒症患者随机分为对照组和治疗组,对照组予以西医常规治疗,治疗组在对常规治疗的基础上予以清毒调血方口服或鼻饲。疗程均为7 d。治疗后进行临床疗效判定、中医证候积分,检测血白细胞计数、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、高迁移率族蛋白B1(HMGB1)水平。结果治疗后2组证候积分较治疗前均有显著下降,治疗组下降较对照组明显(P<0.05)。治疗组总有效率优于对照组(P<0.05)。2组患者治疗后外周血白细胞计数、CRP、TNF-α、HMGB1水平较治疗前均有显著下降,治疗组TN F-α、HMGB1下降幅度较对照组更显著,均P<0.05。结论在常规西医治疗的基础上加用清毒调血方治疗能提高脓毒症患者的临床疗效,降低血清炎症白细胞计数、CRP、TNF-α及HMGB1水平。Objective On the basis of routine Western medicine treatment, we observed the curative effect of the treatment of sepsis with oral or nasal feeding by clearing away toxin and regulating blood, and the influence on inflammatory factors and HGMB1.Methods 70 cases of sepsis patients were randomly divided into the control group and the treatment group. The control group received routine Western medicine treatment, and the treatment group was given the prescription, or oral feeding on the basis of conventional treatment. The course of treatment was 7 days. After treatment, the clinical efficacy was determined and the scores of TCM syndromes were detected. The levels of WBC count, CRP, TNF-α and HMGB1 were detected. Results After treatment, the score of syndromes in the two groups decreased significantly compared with that before treatment, but decreased in the treatment group compared with that in the control group（ P〈0.05）. The total effective rate of the treatment group was better than that of the control group（P〈0.05）. After treatment, the peripheral blood leukocyte count, CRP, TNF-α and HMGB1 levels in the two groups were significantly lower than those before treatment, and the decrease of TNF-α and HMGB1 in the treatment group was more significant than that in the control group（P〈0.05）. Conclusion On the basis of routine western medicine treatment, the treatment of clearing away toxin and regulating blood can improve the clinical efficacy of sepsis treatment, and reduce the levels of serum inflammatory leukocyte count, CRP, TNF-α and HMGB1 levels.
