活血通督汤联合手术治疗退行性腰椎管狭窄症的临床研究

摘　　要：目的:探讨活血通督汤联合手术对退行性腰椎管狭窄症(DLSS)干预的临床疗效。方法:选取甘肃省中医院脊柱骨二科2014年1月至2016年12月共64例退行性腰椎管狭窄症患者,采用随机数字表法分为治疗组32例、对照组32例,两组患者均行后路减压植骨内固定术,治疗组在手术治疗的基础上加用活血通督汤,对照组单纯手术减压植骨内固定。治疗后采用ODI评分、JOA评分及视觉模拟疼痛评分(VAS评分)评定临床疗效。结果:治疗前两组患者的ODI评分、JOA评分及VAS评分比较差异无统计学意义(P>0.05);治疗后两组患者的上述评分均降低,但治疗组显著优于对照组,差异有统计学意义(P<0.05)。结论:基于督脉论治思想,运用活血通督汤联合手术椎管减压,对退行性腰椎管狭窄症(DLSS)可显著缓解患者的疼痛,并有助于患者术后的康复,有较好的近期疗效。Objective：To explore the clinical effect of Huoxue Tongdu decoction combined with surgery for treating degenerative lumbar spinal canal stenosis（DLSS）.Methods：A total of 64 patients with degenerative lumbar spinal stenosis from January 2014 to December 2016 were selected in department of spinal bone II,Gansu provincial hospital of traditional Chinese medicine.Random digit expression was used to divide the patients into treatment group（32 cases）and control group（32 cases）.All of the patients underwent posterior decompression and bone grafting internal fixation.The treatment group was treated with Huoxue Tongdu decoction on the basis of surgical treatment,while the control group was treated with posterior decompression and bone grafting internal fixation alone.After treatment,the ODI score,JOA score and visual analogue pain score（VAS score）were used to evaluate the clinical efficacy.Results：Before treatment,there was no significant difference between the two groups in ODI score,JOA score and VAS score（P〉0.05）.After treatment,the scores of the above two groups were decreased,but in the treatment group significantly better than in the control group,the difference was statistically significant（P〈0.05）.Conclusion：Based on the rule of Du meridian treatment,using Huoxue Tongdu decoction combined with surgical decompression of vertebral canal for treating DLSS patients can significantly relieve the pain and is helpful for the postoperative rehabilitation,with better short-term curative effect,yet without long-term follow-up.
