摘 要:目的比较高通量血液透析与低通量血液透析对维持血液透析患者贫血治疗的影响。方法选择2016年1—7月本院收治的76例维持性血液透析患者为研究对象,入选前均用低通量聚砜膜透析器透析至少3个月,之后患者被随机分为高通量组和低通量组各38例。低通量组患者使用低通量聚砜膜透析器组,高通量组患者使用高通量聚砜膜透析器组,分别观察7个月,在第0个月及治疗后每个月进行随访,记录观察患者血红蛋白(Hb)水平及促红细胞生成素(EPO)用量。结果通量组在更换透析器后Hb逐渐升高,第3、4、5、6、7个月Hb水平均显著高于第0个月,差异有统计意义(P<0.05)。低通量组患者Hb水平无明显变化,其他个月与第0个月相比,差异无统计学意义(P>0.05)。第4、5、6、7个月,高通量组患者的Hb水平显著高于低通量组患者,差异有统计学意义(P<0.05)。高通量组患者EPO用量逐渐减少,从第4个月开始至第7个月明显低于第0个月,差异有统计学意义(P<0.05)。低通量组患者第1~7个月与第0个月EPO用量比较无明显变化,且高通量组患者与低通量组患者同月组间比较也没有明显变化,差异无统计学意义(P>0.05)。结论高通量血液透析纠正维持性血液透析患者贫血优于低通量血液透析。Objective To compare high flux hemodialysis with low flux hemodialysis effects of anemia in patients with maintenance hemodialysis treatment. Methods 76 maintenance hemodialysis patients were selected for the case study by the Blood Purification Center of the West District of Hefei First People ' s Hospital from January to July in 2016. Dialyser with low flux polysulffone membrane was used in all patients for at least 3 months. The patients were then randomly divided in the two groups,a group of low flux polysulfone membrane dialyzer group,another group of high flux polysulfone membrane dialyzer groups,each group of 38 people,7 months observation,respectively at baseline 0 months and each month follow-up after treatment,recording observation patient's hemoglobin level of Hb and EPO.Results In high flux groups,Hb increased gradually,and was significantly higher at the 3th,4th,5th,6th and 7th months after dialyser change than at 0 month( P〈0.05). In low flux group,however,Hb level changed insignificantly compared to the level at 0 month. At the 4th,5th and 6th months after dialyser change,Hb was higher in high flux groups than in lower flux group( P〈0.05).After dialyser change,EPO amount used gradually reduce at the 4th,5th,6th and 7th months( P〈0.05). In low flux group,EPO amount was similar among 7 months. Change of EPO dose was similar among the two groups at the same time points after dialyser change( P〈0.05).Conclusions High flux hemodialysis is better than low flux hemodialysis in maintenance hemodialysis patients.
