摘 要:目的:由肾病综合征脾阳虚证候结合模型大鼠的肾脏水通道蛋白(AQP)表达的改变,探讨疾病和证候的内在联系。方法:将47只SD大鼠随机分为5组:正常组,脾阳虚组,阿霉素肾病综合征组,以及两种病证结合(病证同时和先病后证)的模型组,观测大鼠行为学及体质量改变,造模结束时分别检测各组大鼠尿蛋白,血清白蛋白、血糖、甘油三酯、胆固醇,以及肾脏AQP1、2、4表达情况。结果:造模过程中脾阳虚组、两个病证结合组出现明显脾阳虚症状和体征;与正常组和脾阳虚证候组比较,两个病证结合组尿蛋白显著升高(P<0.01),血清白蛋白显著降低(P<0.05),胆固醇显著升高(P<0.01);与正常组比较,两个病证结合组血糖显著降低(P<0.01,P<0.05),病证同时组甘油三酯显著升高(P<0.01);两个病证结合组AQP1、2、4表达较阿霉素肾病综合征组显著降低(P<0.01)。结论:在肾病综合征基础上,脾阳虚证候的存在会加重疾病,并且会引起全身水液平衡失调,导致肾脏AQP表达减少。Objective: To explore the relationship of disease and syndrome through the changes of aquaporins(AQP) in kidney. Methods: A total of 47 SD rats were randomly divided into 5 group: Control group(Con group), Spleen yang deficiency model group(Spl group), nephrotic syndrome disease group(Adr group), combination of disease and symptoms simultaneously model(Sim group) and disease and symptoms successively group(Suc group). The weights and behavior changes were recorded across the experiment. When the model was completed, 24 hours urine was collected to detect urine protein, the rats' blood was taken to detect albumin, glucose, cholesterol and triglycerides, while kidney tissue was taken to detect AQPs. Results: Spl group, Sim group and Suc group had obvious symptoms of spleen yang deficiency after using the complex factors. Compared with Con group and Spl group, the urine protein and cholesterol of Sim group and Suc group increased significantly(P〈0.01), albumin level and glucose level decreased significantly(P〈0.05). Commpared with Con group, GLU in the Sim group and Suc group was decreased significantly(P〈0.01, P〈0.05). TG in the Sim group was increased significantly(P〈0.01). The results showed that the express of AQP1, AQP2 and AQP4 in Sim group and Suc group were lower than those in the Adr group(P〈0.01). Conclusion: On the basis of nephrotic syndrome, the appearance of syndrome of spleen yang deficiency would degenerate the disease, spleen yang deficiency could lead to disorder of water balance and reduce the expression of AQPs in kidney.
