摘 要:目的探讨婴幼儿非感染性腹泻时乳糖不耐受的发生情况。方法选取2015年3月至2016年12月南京市鼓楼区妇幼保健所儿科门诊就诊的89例非感染性腹泻婴幼儿为观察组,以同期在本所进行健康体检的134例健康婴幼儿为对照组。采用半乳糖氧化酶法检测全部婴幼儿的尿半乳糖,对两组婴幼儿乳糖不耐受情况进行统计学分析。结果观察组婴幼儿乳糖不耐受阳性率为68.50%(61/89),明显高于对照组的19.40%(26/134),差异有统计学意义(P<0.05)。随着年龄的增长,乳糖不耐受的阳性率呈下降趋势。同一年龄段的婴幼儿乳糖不耐受阳性率比较,观察组明显高于对照组(年龄<6个月:80.65%vs 24.49%;6个月≤年龄<1岁:76.92%vs 19.05%;1岁≤年龄<2岁:55.00%vs 16.67%;2岁≤年龄<3岁:41.67%vs 10.53%),差异均有统计学意义(P<0.05)。结论婴幼儿非感染性腹泻时乳糖不耐受的发生率高,对诊断为非感染性腹泻的婴幼儿应进行尿半乳糖检测,以便及时有效地进行干预治疗。Objective To investigate the development of lactose intolerance in infants with non-infectious diarrhea. Methods From March 2015 to December 2016, 89 infants of non-infectious diarrhea were selected as study group, and 134 healthy infants were enrolled as control group. The galactose oxidase method was used to detect urine galactose, and the lactose intolerance in two groups was statistically analyzed. Results The positive rate of lactose intolerance was 68.50%(61/89) in the study group versus 19.40%(26/134) in the control group(P0.01). The positive rate of lactose intolerance was significantly higher in the study group than the control group in a particular age period(P0.05):80.65% vs 24.49% for 0~6 months old infants, 76.92% vs 19.05% for 6 months-1 year old infants, 55.00% vs 16.67% for1-2 years old infants, 41.67% vs 10.53% for 2-3 years old infants. Further analysis showed that the positive rate of lactose intolerance decreased with age. Conclusion The incidence of lactose intolerance in infants with non-infectious diarrhea is high, and urinary galactose detection should be carried out in order to make timely and effective treatment.
