记忆合金聚髌器及聚酯纤维线治疗髌骨下极粉碎性骨折合并髌韧带损伤

摘　　要：目的探讨记忆合金聚髌器及聚酯纤维线治疗髌骨下极粉碎性骨折合并髌韧带损伤的疗效。方法选取2008年5月~2017年6月43例髌骨下极粉碎性骨折合并髌韧带损伤患者,采用记忆合金聚髌器及聚酯纤维线治疗,术后通过关节功能评分及术后X线片对其疗效进行评定。结果本组患者切口均Ⅰ期愈合。住院期间复查X线片见髌骨恢复正常长度,骨折固定牢靠。术后随访时间12~36周,平均24周,随访率88%,复查X线示骨折愈合良好,髌骨位置良好,骨折愈合时间3~6个月,平均4个月。随访期间未出现关节感染、内固定松动及断裂,髌骨未出现脱位。术后膝关节100°~135°,平均125°。所有患者膝关节功能采用胥少汀综合评定方法评定,本组优32例,良6例,可5例,优良率88%。结论记忆合金聚髌器及聚酯纤维线的联合治疗髌骨下极粉碎性骨折合并髌韧带损伤疗效确切,早期的功能训练能够明显改善膝关节的功能。Objective To investigate the clinical effectiveness of the treatment of comminuted fractures of the inferior poles of the patella accompanied by patellar tendon injuries using memory alloy patellar concentrators and polyester sutures. Methods 43 patients of comminuted fractures of the inferior poles of the patella accompanied by patellar ten- don injuries in our hospital from May 2008 to June 2017 were treated with memory alloy patellar concentrators and polyester sutures. The clinical effectiveness was assessed by joint functional score and X-ray after the operation. Re- suits The 1 phrase wound healing rate was 100%. The normal length of patella and the good fixation of the fracture were found when reviewed by using X-ray during hospital stay. The follow-up time after operation ranged from 12 to 36 weeks, averaged 24 weeks. The rate of following up was 88%. Fractures healed well and the position of patella was good. The healing time of fracture ranged from 3 to 6 months, averaged 4 months. There were no joint infections, inter- nal fixation loosening and breakage or pateflar dislocation found. The flexion angle of patella after operation ranged from 100 to 135 degree, averaged 125 degree. The patellar function of all the patients was assessed by using Xu Shaot- ing comprehensive assessment method. 32 patients were assessed as excellent, 6 as good and 5 as fair. The superior rate was 88%. Conclusion The treatment of comminuted fractures of the inferior poles of the patella accompanied by patellar tendon injuries using memory alloy patellar concentrators and polyester sutures was curative. Function training in the early stage would improve the patellar function.
