航天科工集团体检职工血清胱抑素C浓度异常的多因素分析

摘　　要：目的分析北京地区航天科工集团职工体检胱抑素C浓度异常的原因。方法收集体检职工88例的静脉血行胱抑素C检测,记录性别、年龄、体重、体质量指数,体检时血压、血糖、糖化血红蛋白、总胆固醇、甘油三酯、尿酸、天冬氨酸转氨酶,丙氨酸转氨酶;以胱抑素C为因变量,以性别、年龄、体重、体质量指数、血压、血糖、糖化血红蛋白、总胆固醇、甘油三酯、天冬氨酸转氨酶、丙氨酸转氨酶、血尿酸为自变量输入多元线性回归方程。结果多元线性回归分析显示尿酸浓度值、年龄、甘油三酯浓度对血清胱抑素C浓度有显著影响。以胱抑素C 1.2 mg/L为分割点,2组体检人员的体重、尿酸、甘油三酯、体重指数差异有显著意义(P<0.05);胱抑素C浓度与体重、尿酸、丙氨酸转氨酶、甘油三酯、糖化血红蛋白、体重指数有显著正相关关系。结论要减少血清胱抑素C浓度,需要控制体重及血脂,改变生活方式,降低血尿酸值。Objective to investigate the cause of abnormal serum cystatin C concentration for physical examinees of China aerospace science ＆ industry corporation in Beijing district. Methods To collect 88 venous blood samples of physical examinees to test serum cystatin C concentration,together with personal data such as gender,age,body weight, glutamie-pyruvic transaminase,glutamic oxalacetic transaminase,body mass index,blood？pressure,blood sugar, glycosylated hemoglobin,total cholesterol,triglyceride,blood uric acid concentration . Take the serum cystatin C concentration as dependent variable, the personal data （gender,age,body weight,glutamic-pyruvic？transaminase, glutamic oxalacetic transaminase,body？ mass index,blood？pressure,blood sugar,glycosylated hemoglobin,total cholesterol）as independent variable respectively,and establish equation of multiple linear regression by stepwise regre ssion. Results multiple linear regression showed that blood uric acid,age,triglyceride,exerted respectively significant influence on serum cystatin C concentration. Set serum cystatin C 1.2 mg/L as cutoff point, the level of body weight, blood uric acid, triglyceride, body mass index in normal group differed significantly from those of abnormal group;serum cystatin C concentration positively correlated to body weight,blood uric aeid,triglyeeride,body mass index,glutamic-pyruvic transaminase,glycosylated hemoglobin （P〈0.01）.Conclusion In order to lower the serum cystatin C concentration,we must control both body weight and concentration of cholesterol, change lifestyle, reduce blood uric acid.
