摘 要:目的通过改良羊水细胞培养在妊娠中晚期产前诊断中应用,与脐血染色体产前诊断对比分析,探讨妊娠中晚期羊水染色体产前诊断的可行性。方法收集近3年来在我院行产前诊断妊娠中晚期羊水标本共135例以及脐血标本80例。采用改良的羊水细胞培养方法以及正常的脐血培养方法进行染色体核型分析。结果 135例妊娠中晚期羊水细胞培养全部一次性成功,成功率达100%。135例患者中共检出异常染色体核型9例(6.6%)、多态性12例(8.9%)。80例妊娠中晚期脐带血细胞培养成功78例,成功率97.5%,检出异常核型8例(10.3%)、多态性8例(10.0%)。结论改良妊娠中晚期羊水染色体培养成功率高,与脐带血染色体在适用孕周、穿刺并发症、报告时限、异常和多态性染色体检出等方面无明显差异,可较好的满足临床妊娠中晚期孕妇产前诊断的需求。Objective:To explore the feasibility of prenatal diagnosis of amniotic fluid chromosome in the middle and late pregnancy by improving amniotic fluid cell culture in the prenatal diagnosis of late pregnancy,comparative analysis with prenatal diagnosis of cord blood chromosome. Methods:A total of 135 cases of amniotic fluid and 78 umbilical cord blood samples for prenatal diagnosis were collected in our hospital during the past 3 years. A modified amniotic fluid cell culture method and normal umbilical cord blood culture method were used to analyze the karyotype of the chromosome. Results:The successful rate of 135 cases of amniotic fluid cells in the middle and late pregnancy was 100%. Abnormal karyotype was detected in 9 cases(6.6%)and polymorphism in 12 cases(8.9%),and 8 cases abnormal karyotype(10.3%)and 9 cases polymorphism(11.5%)in 78 umbilical cord blood. Conclusion:The improved amniotic fluid chromosome culture has a high success rate in prenatal diagnosis,There was no significant difference with umbilical cord blood karyotype analysis. It can betterly meet the needs of prenatal diagnosis of pregnant women in middle and late pregnancy.
