摘  要:目的探讨早产儿颅内出血高危因素,研究其预防措施。方法选取解放军白求恩国际和平医院新生儿重症监护病房(NICU)2013年1月—2015年12月592例颅内出血早产儿作为出血组,选取同期592例未发生颅内出血的早产儿作为对照组,比较两组一般情况,对颅内出血危险因素进行Logistic回归分析。结果胎龄、出生体重、窒息、机械通气、产前使用地塞米松、胎膜早破、首次血常规异常、羊水异常、分娩方式均为早产儿脑室周围-脑室内(PIVH)的独立危险因素(P<0.05)。结论早产儿PIVH高危因素众多,发病机制复杂,提示临床医生对出生3~7 d早产儿进行筛查,及早诊治,并采取早期相关干预措施。Objective To investigate high risk factors of intracranial hemorrhage in premature infants in order to find preventive strategies. Methods A total of 592 premature infants with intracranial hemorrhage admitted during January 2013 and December 2015 were selected as study group,and other 592 premature infants without intracranial hemorrhage at the same period were selected as control group. General condition was compared in two groups,and high risk factors of intracranial hemorrhage were analyzed by Logistic regression analysis. Results Gestational age,birth weight,asphyxia,mechanical ventilation,antepartum use of Dexamethasone,premature rupture of memberane,first time CRP( after admission the first time of blood routine),amniotic fluid abnormalities and delivery way were independent risk factors of periventricular-intraventricular hemorrhage( PIVH) in premature infants( P < 0. 05). Conclusion Risk factors of PIVH in premature infants are multifactorial,and the pathogenesis is complex and diverse,and therefore clinicians should give screening for premature infants within the 3^(rd)-7^(thd) after birth and treat as early as possible,and related intervention strategies should be performed in early period.
