苍苓止泻口服液治疗儿童腹泻的系统评价

摘　　要：目的:系统评价苍苓止泻口服液治疗儿童腹泻的疗效,为临床合理用药提供循证参考。方法:计算机检索中国期刊全文数据库、中国生物医学文献数据库、万方数据库、中文科技期刊数据库、Pub Med及Cochrane图书馆等,收集苍苓止泻口服液治疗儿童腹泻的随机对照试验(对照组患儿采用常规西药或补液等治疗,观察组患儿仅服用苍苓止泻口服液或在对照组基础上加服苍苓止泻口服液)。提取资料并评价质量后,采用Rev Man 5.3软件进行文献荟萃(Meta)分析。结果:共纳入23篇中文文献,合计3 256例患儿。Meta分析结果显示,观察组患儿的总有效率(OR=5.06,95%CI=3.95~6.50,P<0.001)、轮状病毒转阴率(OR=3.12,95%CI=2.01~4.84,P<0.001)明显高于对照组,平均止泻时间明显短于对照组(OR=-0.47,95%CI=-0.67^-0.27,P<0.001),差异均有统计学意义。结论:苍苓止泻口服液治疗儿童腹泻,可以显著提高总有效率和轮状病毒转阴率,缩短腹泻时间。但限于纳入研究的质量普遍较低,苍苓止泻口服液治疗儿童腹泻的疗效及安全性有待更多大样本、多中心和高质量的随机对照试验证实。OBJECTIVE： To systematically review the efficacy of Canglingzhixie oral liquid in treatment diarrhea in children,so as to provide evidence-based references for clinical rational medication. METHODS： CNKI,CBM,Wanfang,VIP,Pub Med and Cochrane library were retrieved for randomized controlled trials of Canglingzhixie oral liquid in treatment diarrhea in children（ the control group was treated with conventional western medicine or fluid infusion,the observation group was given Canglingzhixie oral liquid alone or that combined with the control group）.Data was extracted and conducted quality evaluation,Rev Man 5. 3 was adopted to carry out Meta analysis. RESULTS：A total of 23 Chinese studies were involved,included 3 256 patients. Results of Meta analysis indicated the total effective rate（ OR = 5. 06,95% CI = 3. 95-6. 50,P〈0. 001）,negative conversion ratio of rotavirus（ OR = 3. 12,95% CI = 2. 01-4. 84,P〈0. 001） of observation group were significantly higher than those of the control group,the average antidiarrheal time of observation group was significantly shorter than that of the control group（ OR =-0. 47,95% CI =-0. 67--0. 27,P〈0. 001）,all with statistically significantly differences. CONCLUSIONS： Canglingzhixie oral liquid can significantly improve the total effective rate and negative conversion ratio of rotavirus in treatment of diarrhea in children,shorten the duration of diarrhea. However,due to the generally low quality of involved studies,the efficacy and safety of Canglingzhixie oral liquid in treatment diarrhea in children still needs verification by randomized controlled trial with larger sample,multi-center and high quality.
