动态测温在精索静脉曲张患者手术前后阴囊温度测量中的应用效果

摘　　要：目的观察动态测温在精索静脉曲张(VC)患者精索静脉曲张手术前后阴囊温度测量中的应用效果。方法将行腹腔镜精索内静脉高位结扎手术治疗的VC患者49例,分为左侧手术组(A组)35例及双侧手术组(B组)14例,选择同期11例健康男性为对照组,采用动态测温仪比较3组术前、术后当天、术后1个月及术后2个月两侧阴囊温度。结果A、B组术前两侧阴囊最低温度(T_(min))、最高温度(T_(max))及平均温度(T_m)均高于对照组(P<0.05),但A组与B组术前两侧阴囊T_(min)、T_(max)及T_m比较,差异无统计学意义(P>0.05)。术后当天,A、B组两侧阴囊T_m、T_(max)及T_(min)均高于术前(P<0.05),且A、B组左侧阴囊T_m、T_(max)及T_(min)比较差异无统计学意义(P>0.05),B组右侧阴囊T_m、T_(max)及T_(min)高于A组(P<0.05)。术后1、2个月,A、B组两侧阴囊T_m、T_(max)及T_(min)均低于术前(P<0.05),且A、B组两侧阴囊T_m、T_(max)及T_(min)比较,差异无统计学意义(P>0.05)。结论腹腔镜下精索内静脉高位结扎术能有效降低VC患者睾丸温度,动态温度测量能够如实反映VC患者手术前后正常生活条件下阴囊温度变化情况。Objective To observe the effect of dynamic temperature measurement applied to scrotum temperature determination before and after surgery for varicocele（VC）.MethodsForty-nine VC patients undergoing laparoscopic high ligation of the internal spermatic veins were divided into 35 cases in the left-lateral operation group（group A）and 14 cases in the bilateral operation group（group B）,and concurrent 11 healthy men as the controls.A dynamic thermometer was used to compare the bilateral temperatures of the scrotum among the three groups before operation,on right postopertive day,after one and two months of operation.ResultsBefore operation,the bilateral scrotum minimum temperature（T_（min））,maximum temperature（T_（max））and mean temperature（T_m）in group A or B were higher than those in the control group（P0.05）,but there was no statistically significant difference between group A and group B in bilateral scrotum T_（min）,T_（max）or T_m（P0.05）.On the right postoperative day,the bilateral scrotum T_m,T_（max）and T_（min）increased in group A and group B compared to those before operation（P0.05）,and no statistically significant difference was observed in left scrotum T_m,T_（max）or T_（min ）between group A and group B（P0.05）.The right scrotum T_m,T_（max）and T_（min）in group B were higher than those in group A（P0.05）.After one and two months of operation,the bilateral scrotum T_m,T_（max）and T_（min）decreased in group A and group B compared to those before operation（P0.05）,and there was no statistically significant difference in bilateral scrotum T_m,T_（max）or T_（min）between group A and group B（P0.05）.ConclusionLaparoscopic high ligation of the internal spermatic veins can effectively reduce the testicular temperature in VC patients,and dynamic temperature measurement can accurately reflect the changes of the scrotum temperature in the normal living conditions of VC patients before and after operation.
