痰热清注射液对老年慢性阻塞性肺疾病伴呼吸衰竭患者肺功能影响及其保护机制探讨

摘　　要：目的：探讨痰热清注射液对老年慢性阻塞性肺疾病（COPD）伴呼吸衰竭患者肺功能影响及其保护机制。方法：选取本院呼吸内科收治的老年COPD伴呼吸衰竭患者174例,按随机数字表法分组,对照组87例予以常规治疗,研究组87例在对照组基础上予以痰热清注射液治疗,检查记录两组间肺功能指标、血气分析指标、血清细胞因子水平、外周血T淋巴细胞水平,同时比较临床疗效。结果：对照组治疗有效率（81.61%）低于研究组治疗有效率（91.96%）,具有统计学意义（P〈0.05）;与对照组比较,研究组治疗后FVC、FEV1、FEV1%水平较高,治疗后动脉血pH、PO2水平较高,PCO2水平较低,治疗后血清TNF-α、IL-8、IL^（-1）0水平较低,TGF-β水平较高,治疗后外周血CD3＋、CD4＋T淋巴细胞、CD4＋/CD8＋水平较高,CD8＋T淋巴细胞水平较低,具有统计学意义（P〈0.05）。结论：痰热清注射液治疗老年COPD伴呼吸衰竭患者的临床疗效确切,能明显改善肺功能和血气分析指标,其肺功能保护机制可能与调节细胞因子水平,改善患者免疫功能,抑制炎症反应有关。Objective： To investigate the effect of Tanreqing Injection on pulmonary function and protective mechanism in elderly patients with chronic obstructive pulmonary disease（COPD） and respiratory failure. Methods： A total of 174 elderly patients with COPD and respiratory failure from the Department of Respiratory Medicine in our hospital were selected and divided into 2 groups： 87 cases in the control group treated with conventional treatment of Western Medicine and 87 cases in the experiment group received Tanreqing Injection on the basis of the control group. The pulmonary function indexes,blood gas analysis indexes,serum cytokine levels,peripheral blood T lymphocyte levels,and the clinical effect were compared after the treatment. Results： The effective rate in the control group（81. 61%） was lower than that of the experiment group（91. 96%） with significant difference（P 0. 05）. Compared with the control group,levels of FVC,FEV1 and FEV1% were higher in the experiment group after treatment; arterial blood levels of pH and PO2 were higher; PCO2 level was lower after treatment; serum levels of TNF-α,IL-8 and IL^（-1）0 were lower; TGF-β level was higher after treatment; peripheral blood levels of CD3＋,CD4＋T lymphocyte and CD4＋/CD8＋were higher; CD8＋T lymphocyte level was lower after treatment with significant difference（P 0. 05）. Conclusion： The clinical effect of Tanreqing Injection treatment in elderly patients with COPD and respiratory failure was exactly. Tanreqing Injection can significantly improve the pulmonary function and blood gas analysis index. The protective mechanism of lung function may be related to the regulation of cytokine levels,the enhancing immunity of patients,and inhibition of inflammatory response.
