摘 要:目的探讨维生素A与新生儿肺炎、败血症及呼吸窘迫综合征的临床相关性。方法选取2016年1月~2017年1月本院收治的新生患儿100例,将患有新生儿肺炎、败血症、呼吸窘迫综合征、肺炎合并败血症的69例患儿作为观察组,将患有高胆红素血症、心肌损伤,凝血功能障碍、低血糖的31例患儿作为对照组,利用高效液相色谱法测定血浆维生素A浓度。结果根据维生素A缺乏的诊断标准,本研究新生儿维生素A缺乏达95.00%(95/100),新生儿维生素A浓度在肺炎、败血症、肺炎合并败血症以及对照组中分别为(0.32±0.11)μmol/L、(0.38±0.15)μmol/L、(0.26±0.09)μmol/L、(0.52±0.18)μmol/L,差异有统计学意义(t=5.277、2.124、3.779,P<0.05);未发现不同分娩方式、性别、胎次对维生素A浓度的影响,未发现维生素A的浓度与新生儿呼吸窘迫综合征、肺炎严重程度以及住院时间有明显相关性。结论新生儿维生素A缺乏发生率较高,且新生儿肺炎与败血症与维生素A缺乏具有相关性,新生儿应更加注重改善维生素A。Objective To investigate the clinical correlation of vitamin A and neonatal pneumonia, septicemia and respiratory distress syndrome (RDS). Methods 100 neonates from January 2016 to January 2017 were selected. 69 children with neonatal pneumonia, septicemia, respiratory distress syndrome, pneumonia combined with septicemia were taken as the observation group, while 31 children with hyperbilirubinemia, myocardial injury, coagulation disorders, hypoglycemia were taken, as the control group. The vitamin A concentration of all cases were assessed by high performance liquid chromatography. Results According to diagnostic criteria of vitamin A deficiency, the rate of vitamin A deficiency among selected cases was 95.00% (95/100), the neonatal vitamin A concentration in pneumonia, septicemia, pneumonia combined with septicemia, control group were (0.32±0.11)μmol/L, (0.38±0.15)μmol/L, (0.26±0.09)μmol/L, (0.52±0.18)μmol/L, with statistical difference (t=5.277, 2.124, 3.779, P〈0.05); The delivery modes, gender, fetal times had no influence on concentration of vitamin A, and the concentration of vitamin A had no significant correlation with respiratory distress syndrome, severity of pneumonia and hospital stays. Conclusion The incidence of neonatal vitamin A deficiency is higher, and neonatal pneumonia and septicemia have correlation with vitamin A deficiency. Therefore newborns should pay more attention to improving vitamin A.
