清热凉血化瘀法配合外用熏蒸对血栓闭塞性脉管炎患者氧化应激及t-PA影响

摘　　要：目的：探讨清热凉血化瘀法配合外用熏蒸对血栓闭塞性脉管炎患者氧化应激及t-PA的影响。方法：选取血栓闭塞性脉管炎患者86例,随机分2组,各43例,对照组采用抗血小板、改善微循环及神经功能等治疗;研究组在此基础上进行中医辨证治疗,采用＂清热凉血、活血化瘀＂治则,内服配合外用熏蒸,持续治疗6周。比较两组患者氧化应激（SOD、NO、ET-1值）和纤溶系统（t-PA、PAI-1、Fbg值）,记录不良反应事件。结果：两组患者不同时间点SOD、NO、ET-1、t-PA、PAI-1、Fbg值上存在差异（P〈0.05）。与治疗前比,两组治疗3周时、治疗6周时SOD、NO值升高,ET-1值降低,PAI-1、Fbg值降低,t-PA值升高（P〈0.05）;与对照组比,研究组治疗3周时、治疗6周时SOD、NO值较高,ET-1值较低,PAI-1、Fbg值较低,t-PA值较高（P〈0.05）。对照组有效率72.09%低于研究组有效率90.70%（P〈0.05）。结论：清热凉血化瘀法内服配合外用熏蒸有利于提高血栓闭塞性脉管炎的治疗效果,通过改善氧化应激和纤溶系统,来促进血液循环,降低血栓形成,是一种有效的治疗方案。Objective：To investigate the effects of clearing heat cooling blood and removing stasis method combined with external fumigation on oxidative stress and t-PA of patients with thromboangiitis obliterans.Methods：86 cases with thromboangiitis obliterans from our hospital were randomly divided into 2 groups,43 cases each,the control group was treated with antiplatelet,to improve microcirculation and nerve function treatment,the study group was treated more with clearing heat cooling blood and removing stasis method combined with external fumigation,and 2 groups were treated for 6 weeks.The oxidative stress（SOD,NO,ET-1）and fibrinolysis system（t-PA,PAI-1 and Fbg）were compared between the two groups,and adverse events were recorded.Results：There were differences in SOD,NO,ET-1,t-PA,PAI-1 and Fbg values at different time points between 2 groups（P0.05）.Compared with before treatment,level of SOD,NO increased,ET-1 decreased,PAI-1 and Fbg decreased,t-PA increased in 2 groups after 3 and 6 weeks of treatment（P0.05）,compared with the control group,the levels of SOD,NO were higher,levels of ET-1 were lower,PAI-1 and Fbg were lower,and levels of t-PA were higher（P0.05）.The effective rate of the control group（72.09%）was lower than the study group（90.70%）（P0.05）.Conclusion：Clearing heat cooling blood and removing stasis method combined with external fumigation in the treatment of thromboangiitis obliterans was effective,and it was helpful to improve the thromboangiitis obliterans treatment effect by improving oxidative stress and fibrinolytic system,could promote blood circulation,reduce thrombosis,which was an effective treatment plan.
