摘 要:目的研究有感染高危因素的围生期孕妇的白细胞数(WBC)、中性粒细胞数、C反应蛋白(CRP)与其所产新生儿相应指标的相关性,为新生儿感染寻找早期诊断指标。方法本研究为前瞻性、横断面研究,选取2017年3~8月在滨州医学院附属医院产科住院的的有感染高危因素的170例围生期孕妇作为研究对象,对孕妇及所产新生儿留取血标本,采用全自动血常规分析仪检测WBC数、中性粒细胞数、CRP。采用Pearson或Spearman分析孕妇WBC数、中性粒细胞数、CRP与新生儿相应指标的相关性。结果 170例孕妇的WBC数、中性粒细胞数、CRP与新生儿相应指标均无相关性(P>0.05)。孕龄≤34周孕妇的WBC数、中性粒细胞数与新生儿相应指标具有相关性(P<0.05),CRP与新生儿CRP无相关性(P>0.05)。孕龄≥34周孕妇的WBC数、中性粒细胞数、CRP与新生儿对应指标均无相关性(P>0.05)。孕龄≤34周孕妇WBC数、中性粒细胞数与新生儿相应指标的升高或降低趋势具有一致性。结论孕龄≤34周有感染高危因素的围生期孕妇,其白细胞数和中性粒细胞数异常可作为新生儿感染的早期诊断指标,对这部分人群要引起重视。Objective To investigate the relationship between the number of white blood cells (WBC),the neutrophils count,the C reactive protein (CRP) and the corresponding indicators of neonatal births in perlnatal pregnant women with high risk factors,and to find early diagnostic indexes for neonatal infection.Methods This study was a prospective and cross-sectional study.In this study,170 pregnant women with high risk factors of infection treated in the obstetrics department of the affiliated hospital of Binzhou Medical University from March to August 2017 were selected as subjects. For pregnant women and newborn babies,the blood samples were collected and the WBC number,neutrophil count and CRP were detected by automatic blood routine analyzer.The Pearson or Spearman method were used to analyze the correlation between WBC number, neutrophils,CRP and neonatal corresponding indexes.Results There was no correlation between WBC number,neutrophil count and CRP of 170 pregnant women and corresponding indexes of neonates (P〉0.05).The WBC number and neutrophils count of pregnant women whose gestational age ≤34 weeks were correlated with the corresponding indexes of neonates (P〈0.05),while there was no correlation between the CRP of pregnant women whose gestational age ≤34 weeks and the CRP of neonates (P〉0.05).There was no correlation between WBC number, neutrophil count and CRP of pregnant women whose gestational age ≥34 weeks and the corresponding indexes of neonates (P〉0.05).The WBC number,neutrophil count and CRP of pregnant women whose gestational age ≤ 34 weeks were consistent with the increase or decrease of the corresponding indexes of neonates.Conclusion The number of white blood cells and neutrophils in perinatal pregnant women with perinatal pregnancy at gestational age ≤ 34 weeks can be used as an early diagnosis index of neonatal infection.This group of people should be paid attention to.
