结构-过程-结果三维质量管理模式在神经内科护理管理中的应用

摘　　要：目的 探索结构-过程-结果三维质量管理模式在神经内科护理质量管理中的应用效果.方法 将基于Donabedian 结构-过程-结果三维质量管理模式应用于神经内科护理质量管理,探索神经内科三维管理模式实施前后科室不良事件发生情况、护士自我效能、护士及患者满意度.结果 实施结构-过程-结果三维质量管理模式后,不良事件发生率为0.179‰,低于实施前的0.418‰,但差异无统计学意义（P〉0.05）,患者满意度得分[（99.3±2.58）分]高于实施前[（96.2±3.16）分],护士对工作的满意度得分[（109.00±17.12）分]较实施前[（77.81±10.31）分]提高,护士的自我效能得分[（29.89± 7.13）分]较实施前[（23.05±5.98）分]提高,差异均有统计学意义（P〈0.01）.结论 实施结构-过程-结果三维质量管理模式减少了不良事件发生率,提高了患者满意度及护士工作满意度,提升了护士的自我效能,是提升临床护理质量的有效方法.Objective To explore the application of structure-process-results 3D quality management model in neurology department for nursing quality management and effect evaluation. Methods The structure-process-results 3D quality management model based on Donabedian was applied on the management of neurology department. Before and after implementation, the adverse event occurrence condition, nurses＇ self-efficacy, satisfaction of nurses and patients were compared. Results After the implementation of structure-process-results 3D quality management model, the adverse event was 0.179‰, lower than 0.418‰ before the model applied, but the difference had no statistical significance （P〉0.05）; the patients＇ satisfaction score was （99.3±2.58）points, which was higher than that before model applied [（96.2±3.16）points]; the nurse＇s work satisfaction score was （109.00±17.12）points, and the nurses＇ self-efficacy score was （29.89±7.13）points, compared with （77.81±10.31）points and （23.05±5.98）points before the model applied, and all the differences had statistical significance （P〈0.05）. Conclusions The structure-process-results quality management model reduce adverse events rate, improve patient＇s satisfaction, nurse＇s work satisfaction and self-efficacy, and contribute clinical nursing quality as an effective method.
