摘 要:目的探讨小儿心电图ST-T改变的病因及临床意义。方法回顾性分析280例患儿心电图伴有ST段抬高、下移、延长、缩短,T波低平、倒置、双向、双峰、增宽、切迹等改变的临床资料,患儿均进行12导联同步常规心电图及部分患儿进行动态心电图检查,并分析其心率变异性(HRV)。结果器质性心脏病原发性ST-T改变141例,占50.36%;其他疾病继发性ST-T改变104例,占37.14%;功能性ST-T改变35例,占12.5%,功能性ST-T改变的HRV时域指标PNN50、r MSSD、SDSD降低,SDNN、SDANN升高。结论小儿心电图ST-T改变病因多种多样,需结合临床综合分析,正确的判断对指导临床治疗有重要意义;HRV可帮助诊断功能性ST-T改变。Objective To investigate the etiology and clinical significance of ECG ST-T changes.Methods A retrospective analysis of 280 cases of patients complicated by ECG STsegment elevation,move down,extend,shorten;T wave flat,inversion,double,twin peaks,notch change clinical data.Patients were 12 lead synchronous ECG and some patients ambulatory electrocardiogram(ECG),and to analyze the heart rate variability(HRV).Results The organic heart disease hair ST-T change in 141 cases,accounting for 50.36%;in 104 cases of other diseases and secondary ST-T change and 37.14%;functional ST-T change in 35 cases,accounted for 12.5%,reduce functional ST-T changes in HRV p NN50,RMSSD,sdsd,SDNN and SDANN were increased.Conclusions Infantile electrocardiogram ST-T change varied etiology,combined with clinical comprehensive analysis and judgment,to guide the clinical treatment has important significance;HRV may help in diagnosis of functional ST-T change.
