某三级综合医院院内真菌感染与环境真菌浓度的关系研究

摘　　要：目的探讨院内真菌感染与空气中真菌浓度与呼吸道病毒感染之间的关系。方法回顾2012年1月至2016年12月住院真菌感染者247例,真菌感染诊断或拟诊时间,以及该月该病区空气真菌培养结果,同时收集这些患者甲型流感病毒、乙型流感病毒、腺病毒和呼吸道合胞病毒等感染情况,在控制混杂因素的条件下,分析空气中真菌浓度与真菌感染发生风险。结果入院在28天以上患者真菌感染与空气中真菌浓度明显相关,在合并甲流和乙流感染时,真菌感染与空气中真菌浓度相关,且感染多发生在入院2周内;合并腺病毒感染时,真菌感染与空气中真菌浓度相关,多发生在入院28天以上者。结论住院超过1个月患者真菌感染与空气中真菌浓度明显相关,呼吸道病毒感染是真菌感染高危因素。Objective The purpose of this study was to explore the relationship among fungal infection in the hospital,the concentration of fungus in air and respiratory virus infection. Methods In retrospect the 247 fungal infected from January,2012 to December,2016 in the hospital,fungal infection diagnosis or suspected time,and monthly airborne fungus culture results in certain ward,meanwhile,having collected infection status of influenza A viruses,influenza B viruses,adenovirus,and Respiratory Syncytial Virus of those patients,under the condition of controlling confounding factors,analyze the occurrence risk of fungal infection related to fungal concentration. Results Fungal infection of the infected patients who had been ill in hospital for more than 28 days obviously correlated with fungus concentration in air.It was related to the fungal concentration in air when those patients co-infected with influenza A viruses and influenza B viruses,and it occured when patients had been in hospital for 2 weeks. So was it when those patients co-infected with adenovirus infection,and the fungal infection occured when patients had been in hospital for more than 28 days.Conclusion Fungal infection in patients with more than one month of hospitalization was significantly associated with the concentration of fungus in the air,and respiratory virus infection was a high risk factor for fungal infection.
