军人非战时颅脑损伤的临床特征分析

摘　　要：目的总结分析8所军队医院军人非战时颅脑损伤(craniocerebral injury,CCI)的临床特征,为其防治提供帮助。方法由神经外科专业人员制定调查表,并培训专业调查人员,计算机检索8所军队医院病历数据库,收集2002-01/2012-12月住院的CCI患者各项临床资料。结果共收集到CCI军人668例,其中男性644例,女性24例;军人干部187例(27.99%),士官135例(20.21%),士兵346例(51.80%)。摔伤355例(53.15%),打击伤143例(21.41%),交通事故伤85例(12.72%),高处坠落伤85例(12.72%)。本组训练时受伤174例(26.05%)。轻型449例(67.22%),中型155例(23.20%),重型51例(7.63%),特重型13例(1.95%)。本研究显示转诊患者427例(63.92%),其中初诊单位为卫生队的193例(28.89%)和地方医院的156例(23.35%)均转诊到部队上级医院。另外,57例(8.53%)患者需要二次转诊。结论非战时军人CCI以轻型CCI为主,主要受伤原因是摔伤,与军事训练有一定关系。Objective To explore the clinical characteristics of servicemen with craniocerebral injury（CCI）in nonwartime.Methods A survey form was made by senior neurosurgeon.Eight surveyors were trained to retrieve the clinical data of sevicemen with CCI who admitted to eight military hospitals between January 2002 and December 2012 in nonwartime.Results There were 668 servicemen with CCI,including 644 males and 24 females.The CCI attributed to tumble,hit,high fall injury and traffic accident were 355（53.15%）,143（21.41%）,85（12.72%）and 85（12.72%）,respectively.The CCI occurred during the military training in 174（26.05%）patients.One hundred and eighty-seven patients（27.99%）were military officer,135（20.21%）were sergeant,and 346（51.80%）were soldiers.Mild,moderate,severe and most severe CCI were 449（67.22%）,155（23.20%）,51（7.63%）and 13（1.95%）,respectively.Four hundred and twenty-seven patients（63.92%）were transferred from one hospital to another during the treatment,and 57（8.53%）were transferred twice.Conclusion Most servicemen with CCI are mild in non-wartime.The main cause of injury is tumble.There is a certain relationship between CCI of servicemen in non-wartime and the military training.
