长期饮酒对老年患者全麻腹腔镜术后认知功能及下肢深静脉血栓形成的影响

摘　　要：目的探讨长期饮酒对老年患者全麻腹腔镜术后认知功能及下肢深静脉血栓形成(DVT)的影响。方法选取2016年10月—2017年3月我院收治的长期饮酒老年患者80例为研究组,选取同期收治非长期饮酒老年患者80例为对照组,两组患者均行全麻腹腔镜手术治疗,对比两组患者的术后认知功能及DVT发生情况。结果手术前两组患者的MMSE(简易精神状态量表)评分无明显区别(P>0.05),手术后不同时间点研究组MMSE评分均显著低于对照组(P<0.05);手术前两组患者的凝血功能无明显区别(P>0.05),手术后研究组患者凝血酶原时间(PT)、活化凝血酶原时间(a PTT)下调、D二聚体(D-D)以及纤维蛋白原(Fbg)等高于对照组;术后DVT发生情况研究组高于对照组(P<0.05)。结论长期饮酒会导致老年患者全麻腹腔镜术后认知功能下降,同时DVT发生风险显著上升。Objective To investigate the effect of long-term drinking on the cognitive function and the formation of deep vein thrombosis （DVT） in the elderly patients after general anesthesia. Methods 80 cases of long-term drinking elderly patients admitted in our hospital from October 2016 to March 2017 were selected as the study group. 80 cases of elderly patients with non-long-term drinking were selected as the control group. All the patients were treated with laparoscopic general anesthesia, Compared the two groups of patients with postoperative cognitive function and DVT occurred. Results MMSE scores of the two groups were significantly different from those of the two groups（P 〉 0.05 ）. The MMSE scores of the study group at different time points after operation were significantly lower than those of the control group （ P 〈 0. 05 ） There was no significant difference in coagulation function between the two groups （ P 〉 0.05 ）. The levels of PT （ prothrombin time）, aPTF （ activated prothrombin time ） were down regulation, D- dimer （ D- D ） and fibrinogen （ Fbg ） were significantly higher in the postoperative group （P 〈 0. 05 ）. The incidence of DVT of the study group was significantly higher than that in the control group （ P 〈 0. 05 ）. Conclusion Long-term drinking can lead to the decrease of cognitive function in elderly patients after laparoscopic general anesthesia, and the risk of DVT is significantly increased.
