摘　　要：目的探讨失效模式与影响分析(fmea)在更年期女性高血压患者中的应用效果及对焦虑状态的影响。方法选择2015年6月-2017年7月河北医科大学第二医院收治的女性更年期高血压患者50例,根据医疗失效模式与影响分析方法,从主题确定、成立fmea小组、绘制更年期女性高血压患者护理流程图、失效模式及潜在风险分析、制定有效的干预措施、分析总结为下阶段实施铺垫等角度实施,采用汉密尔顿抑郁量表(SDS)、焦虑自评量表(SAS)对患者实施fmea干预前、后心理波动进行评估。结果更年期女性高血压患者fmea实施后SAS及SDS评分均低于fmea实施前,差异有统计学意义(P<0.05);失效模式与影响分析实施后全体组员解决问题能力、沟通能力、合作能力、自信心、凝聚力及拓展思维能力均得到明显的提高。同时,医疗失效模式与影响分析的实施能改善患者焦虑状态。结论将失效模式与影响分析用于更年期女性高血压患者中效果理想,有助于降低患者内心的焦虑、抑郁情绪。Objective To explore the application effects of failure mode and effect analysis （fmea） in menopausal women with hyper- tension and the effect on anxiety. Methods From June 2015 to July 2017, 50 menopausal women witb hypertension were selected from the Second Hospital of Hebei Medical University, then they were analyzed from the aspects of theme determination, establishment of fmea group, drawing nursing flowchart, failure mode and analysis on potential risks, making effective intervention measures, laying the foundation for the next phase by analyzing and summarizing data according to medical failure mode and effect analysis. Hamilton Depression Scale （SDS） and Self-rating Anxiety Scale （SAS） were used to evaluate psychological fluctuations before and after fmea intervention. Results SAS and SDS scores after fmea intervention were statistically significantly lower than those before fmea intervention in menopausal women with hypertension （ P〈0. 05 ） . After implementation of medical failure mode and effect analysis, the ability of solving problems, communication, cooperation, self-confidence, cohesion, and expanding thinking of all the members were improved significantly. At the same time, the implementation of medical failure mode and effect analysis can improve anxiety status of patients. Conclusion The effect of medical failure mode and effect analysis is good in menopausal women with hypertension, which can reduce anxiety and depression.
