摘　　要：目的了解北京市性病门诊男性就诊者艾滋病病毒（HIV）感染的流行趋势及感染因素,为评价北京市艾滋病防控效果及制定防治策略提供依据。方法通过横断面调查的方法,对北京市某性病门诊男性就诊者进行行为学调查及血清学检测。结果 2014-2016年共调查1 200人,男性就诊者的HIV抗体阳性率分别为2.75%（11/400）、4.00%（16/400）、6.50%（26/400）,HIV感染率显著上升（趋势χ~2=6.91,P=0.03）。多因素Logistic回归分析显示,最近3个月与临时性伴发生性行为者HIV感染率高于未发生者（P=0.00）[调整比值比（aOR）=6.21,95%可信区间（CI）：1.80~21.35];发生过同性性行为者感染HIV是未发生者的1 349倍（95%CI：326.85~5 566.83）;最近1年诊断为性病者感染HIV风险增加（aOR=4.04,95%CI：1.39~11.74）,以上因素是性病门诊男性就诊者感染HIV的危险因素。结论 2014-2016年北京市性病门诊男性就诊者HIV感染率呈现上升趋势,应加强针对有同性性行为的男性就诊者的干预工作,规范性病门诊的哨点监测工作,为北京市制定艾滋病防治措施提供可靠数据支持。Objective To analyze human immunodeficiency virus（HIV）epidemic and risk factors among male clients to sexually transmitted diseases（SID）clinics,and evaluate the effectiveness of HIV/AIDS prevention and control,and provide evidence for strategy development in Beijing.Methods Cross-sectional surveys were conducted to understand the sexual behaviors and serological test among the clients to STDs clinics.Results From 2014 to 2016,totally 1200 were surveyed,and the HIV infection rates among the clients were 2.75% （11/400）,4.00% （16/400）,and 6.50% （26/400）.The HIV prevalence increased significantly（χ~2=6.91,P=0.03）.Multivariate logistics regression analysis showed that the high risk factors were associated with homosexual intercourse with temporary partners last 3 months,men having sex with men last year,and diagnosed with sexually transmitted diseases.Conclusion HIV infection among STD patients is increasing in Beijing.The intervention of MSM must be strengthened.Sentinel surveillance should be standardized to improve the service quality of STD clinics,so as to provide reliable data for HIV/AIDS prevention and control in Beijing.
