显微镜精索静脉结扎术对精索静脉曲张患者性功能影响的临床研究

摘　　要：目的:探讨显微镜精索静脉结扎术对精索静脉曲张患者性功能影响的的临床研究。方法:选自我院于2014年6月至2016年6月期间收治的精索静脉曲张患者86例,根据手术方法不同分为观察组43例与对照组43例。观察组采用显微镜精索静脉结扎术治疗,对照组采用腹腔镜下精索静脉高位结扎术治疗。结果:观察组手术时间长于对照组,术后鞘膜积液少于对照组,精液质量改善优于对照组,均有统计学差异(P<0.05);两组术中出血量比较无统计学差异(P>0.05);两组术后6个月IIEF-5评分增加,有统计学差异(P<0.05);观察组术后6个月IIEF-5评分高于对照组,且有统计学差异(P<0.05);观察组术后并发症少于对照组,有统计学差异(P<0.05);两组术后6个月泌乳素、卵泡刺激素及黄体生成素下降,有统计学差异(P<0.05);观察组术后6个月泌乳素、卵泡刺激素及黄体生成素低于对照组,有统计学差异(P<0.05)。结论:显微镜精索静脉结扎术治疗精索静脉曲张患者效果明显,且可明显改善患者性功能,具有重要研究意义。Objectives： To investigate the clinical effect of the spermatic cord vein ligation to improve the sexual function. Methods： 86 patients with varicocele were selected in our hospital from June 2014 to June 2016,and according to the different surgical methods,were divided into observation group and control group,each of 43 cases. The observation group was treated with microscope and spermatic vein ligation,while the control group was treated with laparoscopic high ligation of spermatic vein. Results： The operation time of the observation group was longer than that of the control group; postoperative hydrocele was less than the control group; semen quality was better than the control group（ P〈0. 05）. The two group amount of intraoperative bleeding had no significant difference（ P〈0. 05）. The IIEF-5 score increased in both groups 6 months later（ P〈0. 05）,and higher in the observation group than the control group（ P〈0. 05）. The postoperative complications in the observation group were less than the control group（ P〈0. 05）; the prolactin,follicle stimulating hormone and luteinizing hormone all decreased in both groups at 6 months after operation（ P〈0. 05）; the level of prolactin,FSH and LH in the observation group 6 months after were lower than the control group（ P〈0. 05）. Conclusion： The treatment effect of varicocele by microscope and spermatic vein ligation is obvious,which can improve the sexual function of patients.
