四维子宫输卵管超声造影诊断不孕的价值研究

摘　　要：目的:探讨四维子宫输卵管超声造影对不孕症的临床价值。方法:回顾性分析2012年5月至2015年4月共82例不孕症患者,其中41例予腹腔镜检查,41例予四维子宫输卵管超声造影,以腹腔镜结果为金标准,比较诊断符合率、特异度、敏感度等指标变化情况,观察两组不良反应发生率情况。结果:四维子宫输卵管超声造影在输卵管通畅符合率94.52%、狭窄符合率75%、堵塞符合率为80%,总符合率为86.59%,和腹腔镜比较差异无统计学意义(P>0.05);和腹腔镜比较,四维子宫输卵管超声造影敏感度为97.18%、特异度为63.64%,阴性预测值为77.78%、阳性预测值为94.52%;两组不良反应均为7.32%,不良反应发生率比较差异不显著(P>0.05)。结论:四维子宫输卵管超声造影评估不孕症输卵管通畅特异度、敏感度较高,且操作方便简单、可重复,临床价值高。Objectives： To investigate the clinical value of contrast-enhanced ultrasonography in the diagnosis of infertility. Methods： A retrospective analysis on 82 infertility patients from May 2012 to April 2015 was conducted,of which 41 cases were treated with laparoscopy and 41 cases were treated with four-dimensional uterine oviduct ultrasonography. The laparoscopic findings were as the gold standard. The diagnostic accuracy,specificity and sensitivity index changes of two groups were compared,and the adverse effects were observed. Results： In four-dimensional uterine oviduct ultrasonography,the tubal patency coincidence rate was 94. 52%,stenosis coincidence rate was 75%,blocking coincidence rate was 80% and the total coincidence rate was 86. 59%. There was no significant difference compared with the results of laparoscopy（ P〈0. 05）. Compared with laparoscopy,the sensitivity and specificity of four-dimensional uterine oviduct ultrasonography were 97. 18% and 63. 64%,the negative predictive value was 77. 78% and the positive predictive value was 94. 52%; the adverse reactions rate of two groups were both 7. 32%,without significant difference（ P〈0. 05）. Conclusion： The evaluation of four-dimensional uterine oviduct ultrasonography in assessing fallopian tube has high specificity and sensitivity,as a simple and convenient method.
