胆囊腺肌瘤病诊断中超声造影的应用研究

摘　　要：目的探讨超声造影应用于胆囊腺肌瘤病中的诊断价值。方法将我院2015年7月至2016年9月的40例疑似胆囊腺肌瘤病患者作为研究对象。分别实施超声诊断(对照组)及超声造影检查(观察组)。金标准为病理检查。观察两组诊断与病理检查结果对比情况、观察组影像学表现。结果:病理检查胆囊腺肌瘤病67.50%,观察组诊断准确性为77.50%,敏感度为74.07%,较对照组数据而言明显更高,P值小于0.05。超声造影影像学可见弥漫型、局限性以及节段性,其中弥漫型可见胆囊壁未出现肿块,且整体发生肥厚变化;局限性为胆囊壁局部增厚,胆囊内结节、胆囊壁造影显示增强;节段性表现为病灶显影,经显影胆囊壁明显增厚,呈现节段性,高回声区表现为三角形。结论超声造影应用于胆囊腺肌瘤病诊断中,临床价值较高,准确度及灵敏度均较高,具有典型影像学表现。Objective To explore the diagnostic value of contrast-enhanced ultrasound in adenomyoma of gallbladder disease. Methods In our hospital from July 2015 to September 2016 of 40 suspected cases of adenomyomatosis of the gallbladder patients as the research object. The implementation of ultrasound diagnosis respectively （ control group） and contrast-enhanced ultrasound （ obser- vation group）. The gold standard for the observation of pathological examination. Two groups of diagnosis and pathological examination results of the observation group, the imaging findings. Results The pathological examination of adenomyomatosis of the gallbladder in 67. 50% observation group, the diagnostic accuracy was 77.50% , sensitivity was 74. 07% , compared with the control group data were significantly higher and the P value is less than 0. 05. of contrast-enhanced ultrasound imaging showed diffuse type, limitations and segments among them, visible diffuse gallbladder wall without tumor, and overall hypertrophy changes; limitations for local thickening of gallbladder wall in the gallbladder, gallbladder wall nodules, angiography showed enhancement ; segmental lesions show Developing, the development of gallbladder wall thickening, showing segmental, high echo area is triangular. Conclusion The application of con- trast-enhanced ultrasound in the diagnosis of gallbladder adenomyomatosis, clinical value, accuracy and sensitivity are high, with typi- cal imaging findings.
