自体回收血与库存血对前置胎盘剖腹产术后炎性指标的影响

摘　　要：目的:探讨自体回收血与库存血对前置胎盘剖腹产术后炎性反应的影响。方法:选取择期行剖腹产术的前置胎盘患者80例,随机分为自血组(40例)与库血组(40例)。自血组输注经血液回收机回收的自体血,库血组按需输注库存血。比较回收血与库血血红蛋白(Hb)、血细胞压积(Hct)及红细胞三磷酸腺苷(ATP)和2,3-二磷酸甘油酸(2,3-DPG)含量。记录并比较2组术前(T0)、术后1d(T_1)、术后3d(T_2)、术后7d(T_3)外周血白细胞(WBC)数量、C反应蛋白(CRP)水平和红细胞沉降率(ESR)。结果:(1)自体回收血Hb、Hct、ATP及2,3-DPG水平明显高于库存血,差异有统计学意义(P<0.05)。(2)与T0时比较,2组患者T_1、T_2时外周血WBC计数明显升高,T_1、T_2、T_3时CRP、ESR明显升高,差异有统计学意义(P<0.05);与自血组同时点比较,库血组T_1、T_2、T_3时CRP、ESR明显升高,差异有统计学意义(P<0.05)。结论:与库存血相比,自体回收血的质量较好,其输注对前置胎盘剖腹产术后炎性反应的影响较小。Objective：To explore the different impacts of salvaged blood autotransfusion and banked blood transfusion on inflammatory indicators in the cesarean section patients with placenta previa.Method：A total of 80 cases of cesarean section patients with placenta previa were divided randomly into autologous blood group and banked blood group,40 cases in each group.The patients in autologous blood group were infused the salvaged autologous blood by blood recycling machine,and the patients in banked blood group were infused the banked allogeneic blood.Hemoglobin（Hb）,hematocrit（Hct）,RBC adenosine triphosphate（ATP）and 2,3-diphosphate glyceric acid（2,3-DPG）between the autologous blood and the banked allogeneic blood were compared.Number of white blood cells（WBC）,levels of c-reactive protein（CRP）and erythrocyte sedimentation rates（ESR）of peripheral blood preoperative（T0）,postoperative 1 d（T1）,postoperative 3 d（T2）and postoperative 7 d（T3）in two groups were recorded and compared.Result：（1）Hb,Hct,ATP and 2,3-DPG in the autologous blood were higher obviously than those in the banked blood,the differences were statistically significant（P〈0.05）.（2）Compared with T0,WBC of peripheral blood at T1 and T2 increased significantly and CRP,ESR at T1,T2 and T3 elevated obviously in two groups,and the differences were statistically significant（P〈0.05）.Compared with autologous blood group,CRP and ESR elevated obviously in banked blood group,and the differences were statistically significant（P〈0.05）.Conclusion：Compared with banked blood,the salvaged autologous blood might have better quality and little impact on inflammatory reaction in cesarean section patients with placenta previa.
