改良PPH治疗急性嵌顿性混合痔的临床研究

摘　　要：为探讨改良PPH治疗急性嵌顿性混合痔的临床疗效,将80例急性嵌顿性混合痔患者随机分为治疗组和对照组,各40例.治疗组采用改良P P H治疗,对照组采用传统外剥内扎术治疗.对比2组患者近期总体疗效、手术时间、术中出血量、创面愈合时间、术后并发症和远期疗效.结果显示,治疗组总有效率（100%）明显高于对照组（900.%）,P〈0.01;治疗组手术时间、创面愈合时间均明显短于对照组,术中出血量明显少于对照组,术后疼痛评分明显低于对照组,P〈0.05;治疗组术后肛缘水肿、肛门狭窄发生率及6-12个月复发率均低于对照组,P〈0.05;治疗组满意度高于对照组,P〈0.05;2组在术后出血、精细控便力下降方面比较差异无统计学意义,P〉0.05.结果表明,改良PPH治疗急性嵌顿性混合痔临床疗效确切,对肛门精细控便能力无影响,手术安全可行,手术时间短,术中及术后出血少,创面愈合快,患者满意度高,值得临床应用.This study was to explore the clinical effect of im mixed hemorrhoids,randomly divided 80 cases o 40 cases for each, which the former adopted the ssection internal ligation; then compare this hemorr improved PP oth group＇s bleeding volume, wound surface healing time, pos except m postoperative bleeding, exactly control a H,while the latt whole effect, optoperative compl bility on feces th ication and 1 ere was no s both groups（ P 〉0.05） ;in the former total effective rate （100% vs 90.0%0） face healing time,intraoperative bleeding volume,pain score after surgery, ,op pos atmg acute mcarce group and control rated one, er received routine external erative time, intraoperative ong-term effect. As results, tatistical difference between erative time and wound sur- toperative incidence of ano- edge edema and anal stenosis as well as recurrence rate during 6-12 months after surgery,patient＇s satisfac tlon （P degree were respectively higher, shorter, less, lower, lower, higher than that in the latter significantly （P〈0.01,or P 〈0.05）.Above results show that the improved PPH for acute incarcerated mixed hemor ds has certain clinical effect, no influlence on exactly control feces of anus,and performing operation also is safe an face, as well Key Words d feasible, of shorter time, less post-and intraoperative as higher patient＇s satisfactory degree, Acute incarcerated mixed hemorrhoid thus it is worth bleeding, faster healing of wound sur to be popularized to use clinically.
