手术联合中药熏洗治疗婴幼儿肛瘘的临床研究

摘　　要：为探讨手术联合中药熏洗治疗婴幼儿肛瘘的临床疗效,对36例肛瘘患儿行手术治疗,术后予以中药熏洗肛门局部（观察组）,观察患者手术疗效、并发症、创面愈合时间、复发情况等,并选择同期于我院3家合作单位（汉中市中医医院、汉中市人民医院、汉中市3201医院）就诊接受保守治疗的36例患儿（对照组）进行对比分析.结果显示,观察组痊愈率、总有效率均明显高于对照组,P〈0.05;观察组治疗后肛门疼痛程度明显轻于对照组,肛缘水肿发生率明显低于对照组,P〈0.05;观察组治疗时间和创面痊愈时间均明显短于对照组,P〈0.05;治疗后6个月,观察组复发率明显低于对照组,P〈0.05.观察组术后1个月后肛门功能、肛门指诊情况、盆底肌电图及肛管直肠压力测定均恢复正常.结果表明,手术联合中药熏洗治疗婴幼儿肛瘘安全可行,疗效确切,复发率低,值得临床推广应用.In order to explore the clinical effect of surgery combined with TCM fuming 〉 washing for trea ting infantile anal fistula,36 cases of fistula of anus received surgical treatment,afterwards local TCM fu- ming ga washing,as observation group;at same time another 36 cases（from 3 cooperative hospitals） undergoing conservative treatment were taken as control group;then both group＇s following status such as operative effect,complication, wound-surface healing time, and recurrence, etc were compared and analysed. As results,in curative rate and total effective rate, in postoperative pain degree of anus, in ano-edge edema inci- dence,in the time for treatment and for wound-surface cured, and in recurrence rate at 6 months after treat- ment, the former was significantly higher, milder, lower, shorter and lower than the latter（all P 〈0.05） ; at 1 month after surgery the anal function,digital diagnosis status of anus,EMG inspection of pelvic floor,and ano-rectal pressure measurements just returned normal.Results show that surgery combined with TCM fuming washing for treating infantile anal fistula is safe and feasible,of certain effect and low recurrence rate,thus worth to be popularized to use clinically.
