摘 要:目的:探讨功能磁共振检查技术在颞叶癫痫脑内病变检测中的临床应用价值。方法:2015年1月至2016年12月在我院诊断为颞叶癫痫患者50例,年龄在10~56岁,平均21.3±10.3岁。使用GEsigna1.5及3.0THDxt磁共振机,常规扫描:垂直于海马的斜冠状位T2及T2 FLAIR,轴位T1 FLAIR、T2及T2FLAIR;在常规扫描基础上加扫轴位高分辨SWI及MRS;图像在GE ADW4.4工作站处理,采用SPSS19.0进行处理分析,经卡方检验,结果以P<0.05认为差异有统计学意义。结果:50例患者中,常规磁共振发现颅内病灶32例(32/50),常规磁共振扫描基础上加SWI扫描发现颅内病灶37例(37/50),MRS谱线异常41例(41/50),未见异常9例(9/50)。常规磁共振联合功能磁共振扫描对病变的显示能力得到提高,P<0.05,两者具有显著差异。结论:颞叶癫痫脑内病变影像学表现常不十分明显,因此在磁共振检查中应注意加扫功能磁共振成像,有利于脑内病变的检出。Purpose:To investigate the clinical value of functional magnetic resonance imaging in detecting of temporal epileptic fool. Methods: Clinically confirmed 50 cases with temporal epilepsy were analyzed in our hospital from January 2015 to December 2016. The mean age was 21.3±10.3 years, and ranged from 10 to 56 years. All these patients underwent MRI examination on GE signal.5T and 3.0T HDxt scanner, the conventional MRI sequences included oblique coronal T2WI and FLAIR T2WI, axial FLAIR T1WI, T2WI and FLAIR T2WI sequences, and oblique coronal T2WI FLAIR was taken in oblique section vertical to the hippocampus major axis. The functional magnetic imaging sequences included MRS and axial high resolution SWI, and then the original images were transmitted to the GE ADW4.4 workstation for image processing. The detection of the lesions by conventional MRI and fMRI was compared. All the data were analyzed by chi square test with SPSS 19.0 software, and P〈0.05 was set as the statistical significant. Results: In all 50 patients, 32 lesions were detected by conventional MRI, and 37 lesions were detected by SWI. MRS spectrum abnormalities were found in 41 patients. Conclusion: Owing to the temporal lobe is close to the base of the skull and the adjacent structures are complex, it is often difficult to detect lesions. So we should pay attention to the functional magnetic resonance imaging which is conducive to the detection of temporal lobe lesions and the assessment of the disease.
