Quantitative Analysis of Lumbosacral Nerve Roots of Normal Adults with Magnetic Resonance Diffusion Tensor Imaging

摘  要:目的:探讨正常成人L4、L5、S1神经根磁共振弥散张量成像(DTI)和神经纤维束示踪成像(DTT)的可行性及量化特征。方法:20名正常成人志愿者接受3.0T多源磁共振检查,采用选择性激励技术(PROSET)及DTI融合图像,分别在L4、L5、S1神经根的节前神经根鞘硬膜囊出行处、神经节和节后脊神经前支出椎间孔外口处三个不同水平设定兴趣区(ROI)测量左、右两侧神经根的各向异性分数(FA)值和表观弥散系数(ADC)值,同时进行神经根DTT。结果:20名正常成人志愿者PROSET、DTI及DTT均成功,可以清晰显示L4、L5、S1神经根,测量其FA及ADC的均值分别为0.56±0.10和(0.94±0.53)mm^2/s,0.52±0.11和(1.07±0.50)mm^2/s,0.48±0.11和(1.18±0.45)mm^2/s;各神经根相同水平左、右两侧的FA值及ADC值差异均无统计学意义(P>0.05);各神经根ADC值除节前神经根鞘硬膜囊出行处水平差异无统计学意义外,其他水平各神经根之间的FA值、ADC值差异均有统计学意义(P<0.05),且FA值变化呈L4>L5>S1趋势,ADC值变化趋势反之;同一神经根在三个不同水平的FA值、ADC值差异均有统计学意义(P<0.05),且FA值变化呈节前<神经节<节后趋势,ADC值变化趋势反之。结论:正常成人L4、L5、S1神经根DTI及DTT是可行的,与PROSET融合图像能够显示神经根走行并进行量化分析,为研究神经根病变提供了较全面的影像学信息。Purpose: To investigate the feasibility of diffusion tensor imaging (DTI) and diffusion tensor tractography (DTT) in evaluation of L4, L5, S1 nerve roots of normal adults. Methods: Twenty healthy volunteers were studied with Philips Achieva 3.0T magnetic resonance scanner. Principle of selective excitation technique (PROSET) and DTI fusing images were used. Apparent diffusion coefficient (ADC) and fractional anisotropy (FA) of region of interest (ROI) on three different levels of bilateral L4, L5 and S1 nerve roots were measured, DTT were also performed. Results: Twenty volunteers were successful reconstructed by PROSET, DTI and DTT. L4, L5, S1 nerve roots could be shown clearly on PROSET and the DTT map. Average FA and ADC values of healthy volunteers on IA, L5, S 1 were 0.56±0.10 and (0.94±0.53)mm^2/s, 0.52±0.11 and (1.07±0.50)mm^2/s, 0.48±0.11 and (1.18±0.45)mm^2/s, respectively. There wasno significant difference of diffusion tensor parameters between bilateral nerve roots (P〉0.05). There were significant differences of FA and ADC values among LA, L5 and S1 nerve roots at the same level (P〈0.05). The variation of FA value was with L4 〉L5 〉S1 trend, while the ADC value was with L4 〈L5 〈S1 trend. There were significant differences of FA and ADC values among three different levels of the same nerve root (P〈0.05). The variation of FA value was with pre-ganglion 〉ganglion 〉post-ganglion trend, while the ADC value with pre-ganglion 〈ganglion 〈post-ganglion trend. Conclusions: DTI and DTT on L4, L5, S1 nerve roots of normal adults are feasible. PROSET, DTI and DTT fusing images can be used to display the architecture of the nerve roots and provide quantitative information for further research of the nerve root diseases.
