尤瑞克林联合达比加群酯治疗房颤合并脑栓塞的临床疗效

摘　　要：目的探讨尤瑞克林联合达比加群酯治疗心房颤动(房颤)合并脑栓塞的临床疗效。方法 76例房颤合并脑栓塞患者,随机分为对照组和观察组,各38例。对照组采用尤瑞克林治疗,观察组采用尤瑞克林联合达比加群酯治疗,比较两组患者的临床疗效、凝血功能指标、神经功能变化情况。结果观察组治疗总有效率为92.11%,明显高于对照组的71.05%,差异有统计学意义(P<0.05);观察组治疗后血浆凝血酶原时间(PT)、血浆活化部分凝血活酶时间(APTT)、血浆凝血酶时间(TT)均长于对照组,国际标准化比值(INR)明显高于对照组,差异有统计学意义(P<0.05);观察组治疗后美国国立卫生研究院卒中量表(NIHSS)评分和日常生活活动能力量表(BI指数)评分明显优于对照组,差异有统计学意义(P<0.05)。结论尤瑞克林联合达比加群酯治疗房颤合并脑栓塞疗效显著,可有效改善患者凝血功能,促进神经损伤的修复,且不增加出血风险,具有积极的临床意义。Objective To discuss the clinical efficacy of urinary kallidinogenase combined with dabigatran etexilate in the treatment of atrial fibrillation complicated with cerebral embolism.Methods A total of 76 patients with atrial fibrillation complicated with cerebral embolism were randomly divided into control group and observation group,with 38 cases in each group.The control group was treated with urinary kallidinogenase,and the observation group was treated with urinary kallidinogenase combined with dabigatran etexilate.Comparison were made on clinical efficacy,coagulation parameters and neurological changes between the two groups.Results The observation group had obviously higher total treatment effective rate as 92.11% than 71.05% in the control group,and the difference was statistically significant（P〈0.05）.The observation group had obviously longer plasma prothrombin time（PT）,plasma activated partial thromboplastin time（APTT）,thrombin time（TT） than the control group,and higher international normalized ratio（INR） than the control group.Their difference was statistically significant（P〈0.05）.The observation group had obviously better National Institutes of Health Stroke scale（NIHSS） score and daily living ability scale（BI index） score than the control group,and their difference was statistically significant（P〈0.05）.Conclusion Combination of urinary kallidinogenase and dabigatran etexilate shows remarkable efficacy in the treatment of atrial fibrillation complicated with cerebral embolism,and it can effectively improve the blood coagulation function,promote the repair of nerve injury without increasing the risk of bleeding.It has positive clinical significance.
