H型高血压患者动态血压变异性及动脉硬化特点的临床研究

摘　　要：目的探讨H型高血压患者动态血压变异性及动脉硬化特点,为H型高血压防治提供依据。方法选取2015年3月~2017年3月于沧州中西医结合医院心内科收治的原发性高血压患者120例。所有患者均接受血浆同型半胱氨酸(Hcy)水平测定,根据血浆Hcy水平分为H型高血压组(Hcy≥10μmol/L,n=68),非H型高血压组(Hcy<10μmol/L,n=52)例。分别检测两组患者的各项动态血压变异性情况以及动脉硬化指标,应用Pearson相关性分析与心-踝血管指数(CAVI)的相关性。结果 H型高血压患者血浆Hcy水平为显著高于非H型高血压(P<0.05)。H型高血压组患者的24 h平均收缩压标准差(24h SSD)、日间收缩压标准差(d SSD)、夜间收缩压标准差(n SSD)、夜间舒张压标准差(n DSD)、24h收缩压加权标准差(24h SwSD)以及24 h舒张压加权标准差(24h DwSD)水平均显著高于非H型高血压组(P<0.05)。H型高血压组患者的L-CAVI、R-CAVI水平均显著高于非H型高血压组(P<0.05)。经Pearson相关性分析显示:H型高血压患者24h SSD、d SSD、n SSD、n DSD、24h SwSD以及24h DwSD均与CAVI水平呈正相关关系(P<0.05)。结论 H型高血压患者动态血压变异性增高,且动脉硬化程度增大。Objective To study the characteristics of dynamic blood pressure variability and atherosclerosis in H-type hypertension patients. Methods 120 patients of primary hypertension were selected from March 2015 to March 2017. All patients received plasma homocysteine （Hcy） level. 68 patients with h-type hypertension group according to Hcy level, 52 patients with non-h type hypertension group. The dynamic blood pressure variability and arteriosclerosis index were measured in the two groups, and Preason correlation analysis was performed. Results The Hcy level of H-type hypertension was significantly higher than that of non-H-type hypertension （P〈0.05）. H-type hypertension group 24h SSD, dSSD, 24h SWSD nSSD, nDSD, and 24h DWSD level was respectively was significantly higher than that of the non- H type hypertension group（P〈0.05）.H hypertension group patients with L-CAVI, R - CAVI level was significantly higher than that of the H hypertension group （P〈0.05）. According to Preason correlation analysis, 24h SSD, dSSD, nSSD, nDSD, 24h SwSD and 24h DwSD were positively correlated with CAVI level （all P〈0.05）.
