主动脉内球囊反搏在左室室壁瘤手术患者中的应用效果

摘　　要：目的探讨主动脉内球囊反搏(IABP)在左室室壁瘤手术患者围手术期应用的效果。方法回顾性分析1999年1月~2012年4月于阜外医院接受左室室壁瘤手术的282例患者资料,男性221例,女性61例。根据围手术期是否应用IABP,将患者分为IABP组(n=31)和对照组(n=251)。对比分析两组术前基线资料,包括年龄、性别比、冠状动脉病变情况、体外循环时间、阻断时间以及超声指标;术后住院死亡率、并发症发生率、呼吸机使用时间、ICU停留时间、术后住院时间、住院费用等。结果两组年龄、性别比、冠状动脉病变情况、体外循环时间及阻断时间等比较,差异无统计学意义(P均>0.05)。与对照组比较,IABP组左室射血分数降低,左室舒张末期内径增加,应用正性肌力药物比例增加,差异有统计学意义(P均<0.05)。两组住院死亡率、神经并发症、严重感染、总并发症、呼吸机使用时间、ICU停留时间、术后住院时间、住院费用等方面比较,差异无统计学意义(P均>0.05)。结论左室室壁瘤手术患者围手术期应用主动脉内球囊反搏安全有效,临床效果满意。Objective To investigate the perioperative application effect of intra-aortic balloon pump （IABP） in patients undergone left ventricular aneurysm surgery. Methods The materials of patients （n=282, male 221 and female 61） undergone left ventricular aneurysm surgery were retrospectively analyzed in Fuwai Hospital from Jan. 1999 to Apr. 2012. All patients were divided, according to whether or not IABP was applied during perioperative period, into IABP group （n=31） and control group （n=251）. The baseline data including age, sex ratio, coronary artery lesions, time of cardiopulmonary bypass （CPB）, cross clamp time and ultrasound indexes, postoperative in-hospital mortality, incidence of complications, application time of respirator, ICU stay time, postoperative hospital stay time and total hospitalization costs were compared and analyzed in 2 groups. Results The difference in age, sex ratio, coronary artery lesions, CPB time and cross clamp time had no statistical significance between 2 groups （all P〉0.05）. Compared with control group, left ventricular ejection fraction （LVEF） decreased, left ventricular end-diastolic inner diameter （LVEDd） increased and percentage of patients taken positive inotropic drugs increased in IABP group （all P〈0.05）. The difference in in-hospital mortality, neurological complications, serious infection, total complications, application time of respirator, ICU stay time, postoperative hospital stay time and total hospitalization costs had no statistical significance between 2 groups （all P〉0.05）. Conclusion IABP applied during perioperative period is safe and effective with satisfied clinical efficacy in patients undergone left ventricular aneurysm surgery.
