糖尿病终末期肾病患者肾移植治疗的临床研究

摘　　要：目的探讨患糖尿病的终末期肾病患者行肾移植治疗的临床特征。方法回顾性分析2009年1月至2013年1月在解放军第153中心医院接受肾移植的408例患者的临床资料，根据患者术前是否患有糖尿病将其分为糖尿病组（82例）和非糖尿病组（326例），分析比较术后感染、移植肾功能延迟恢复（DGF）、不良事件的发生情况，比较受者和移植肾存活率的差异。结果糖尿病组和非糖尿病组术后感染率分别为23.2%（19/82）和15.6%（51/326），差异有统计学意义（P＝0.04）；DGF发生率分别为17.1%（14/82）和8.6%（28/326），差异有统计学意义（P＝0.04）；不良事件发生率分别是13.4%（11/82）和8.3%（27/326），差异有统计学意义（P＝0.03）；两组受者和移植肾1、2、3年存活率的差异无统计学意义（P〉0.05）。结论糖尿病终末期肾病患者行肾移植治疗后感染、DGF、不良事件发生率增高，术后要加强对这类并发症的防治，妥善处理，受者和移植肾近、中期存活不受影响。Objective To explore the clinical characteristics of renal transplantation in patients with diabetes mellitus （DM）-induced end-stage nephropathy. Methods The clinical data of 408 cases who underwent renal transplantation in our center from 2009 to 2013 were retrospectively analyzed. The patients were divided into DM group （n = 82） and non-DM group （n = 326）. The postoperative infection, delayed graft function （DGF）, adverse events, and the survival rate of patients/kidneys were comparatively analyzed. Results The incidence of postoperative infection, DGF and adverse events was significantly higher in DM group than in non-DM group （23.2%o vs. 15.6%, P = 0. 04; 17. 1% vs. 8. 6%, P = 0. 04% 13.4% vs. 8. 3%, P = 0. 03）. No significant difference was found in the 1-, 2-, and 3-year survival rate of patients and kidneys between the two groups after operation （P〉 0. 05）. Conclusion The incidence of postoperative infection, DGF and adverse events is higher in DM patients. The DM does not affect the survival rate of patients/kidneys through appropriate treatment. It is important to prevent complications in DM patients after renal transplantation.
