高密度毛发移植术在瘢痕性脱发修复中的应用

摘　　要：目的探讨高密度毛发移植术在瘢痕性脱发修复中的应用效果及其特点。方法选取2014年3月至2015年6月期间延安大学附属医院烧伤整形手外科收治的80例瘢痕性脱发患者,对其实施种植密度在20~30 FU/cm^2之间的高密度毛发移植术,术后定期回访6个月,观察其治疗效果,对比患者治疗前后满意度、焦虑抑郁程度以及生活质量改善情况。结果手术结束后,患者脱发区大部分被覆盖,且患者移植后的毛发形态较为自然,所有患者对术后效果满意。手术后对患者随访6个月,患者脱发区完全覆盖者35例(43.75%),基本覆盖者25例(31.25%),未覆盖者15例(18.75%),5例患者伴有头皮瘢痕明显外露,3例未覆盖患者发生表皮样囊肿,经过局部清创处理后,患者均愈合。治疗后患者的满意度评分为(94.09±1.05)分,明显优于治疗前的(73.51±2.01)分,差异有统计学意义(P<0.05);治疗后患者的焦虑和抑郁程度及生活质量改善评分分别为(42.59±1.09)分、(35.79±1.24)分、(94.25±3.05)分,均明显优于治疗前的(65.14±1.13)分、(57.86±1.21)分、(72.24±3.12)分,差异均有统计学意义(P<0.05)。结论高密度毛发移植术在瘢痕性脱发修复中具有显著的治疗效果,且毛发生长形态自然、成活率高、移植密度高、恢复快、创伤小。Objective To explore the application effect and characteristics of high-density hair transplantation in the repair of cicatricial alopecia.Methods Eighty patients with cicatricial alopecia who admitted in Department of Burns and Plastic Surgery,Affiliated Hospital of Yan＇an University from March 2014 to June 2015 were selected.All patients were treated with high-density hair transplantation between 20 FU/cm2 and 30 FU/cm2.The patients were followed up regularly for 6 months,and the therapeutic effect were observed.The satisfaction,anxiety,depression and quality of life of the patients before and after treatment were compared.Results After the operation,most hair loss areas were covered,and the hair morphology of the patients after transplantation were natural.All patients were satisfied with the postoperative effect.The patients were followed up for 6 months after the operation.The hair loss areas were completely covered in 35 cases（43.75%）,mostly covered in 25 cases（31.25%）,and not covered in 15 cases（18.75%）,and 5 patients had obvious scalp scar exposure.Epidermoid cyst occurred in 3 patients.After treatment,the satisfaction score of patients was（94.09±1.05）,which was significantly better than（73.51±2.01） before treatment,P0.05.After the treatment,the anxiety,depression and quality of life improvement scores were respectively（42.59±1.09）,（35.79±1.24）,（94.25±3.05）,which were significantly better than（65.14±1.13）,（57.86±1.21）,（72.24±3.12） before treatment,P0.05.Conclusion High density hair transplantation has significant therapeutic effect in the treatment of Cicatricial Hair Loss,which had natural hair growth,high survival rate,high transplanting density,quick recovery,and small trauma.
