炎症因子与静脉血栓栓塞症的相关性研究

摘　　要：目的探讨血清淀粉样蛋白A(SAA)、C-反应蛋白(CRP)等炎症因子水平与静脉血栓栓塞症(venous thromboembolism,VTE)的关系。方法选择2016年1月-2017年6月在新疆医科大学第一附属医院住院确诊VTE的患者230例作为病例组,以同期住院的体检无VTE、且性别及年龄相匹配的患者184例作为对照组,应用酶联免疫吸附法分别检测2组患者血清SAA、CRP、IL-6、TNF-α水平并进行比较。结果 VTE组吸烟者的比例和腰围[(83.31±10.13)vs(80.01±10.64)cm)]均高于对照组,且差异有统计学意义(P<0.05)。VTE组的甘油三酯[(1.90±0.99)mmol/L]高于正常对照组[(1.59±1.14)mmol/L],差异有统计学意义(P<0.05)。VTE组IL-6、TNF-α、CRP、SAA均升高,与对照组比较,差异均有统计学意义(P<0.05)。经非条件logistic回归分析,校正后,SAA、IL-6、TNF-α、高脂血症不是VTE的独立危险因素,而CRP水平、腹型肥胖、吸烟是VTE的危险因素。结论炎症因子与静脉血栓栓塞症密切相关,可用以辅助临床诊断和治疗。Objective To investigate the relationships between inflammatory factors levels and venous thromboembolism(VTE).Methods 230 patients diagnosed as VTE in Department of Cardiac Failure of the First Affiliated Hospital of Xinjiang Medical University from January 2016 to June 2017 were enrolled as case group.During the same period,184 inpatients without VTE matched in gender and age with the caseswere as control group.The enzyme-linked immunosorbent assay were used to detect the blood serum SAA,CRP,IL-6,TNF-αlevels.Results In VTE group,smoking and waist circumference[(83.31±10.13)vs(80.01±10.64)cm)]were higher than those in the control group,and the difference was statistically significant(P <0.05).TG level[(1.90±0.99)mmol/L]in VTE group was higher than that in control group[(1.59±1.14)mmol/L].IL-6,TNF-α,CRP,SAA levels in VTE group were all higher than those in the control group,and the difference was statistically significant(P <0.05).CRP,abdominal obesity,smoking were independent risk factors for VTE patients,while SAA,IL-6,TNF-α,hyperlipidemia were not independent risk factor by logistic regression analysis.Conclusion Inflammatory factors are associated with venous thromboembolism,which can be used to clinical assist diagnosis and treatment.
