成人房间隔缺损封堵术后左心大小和功能的变化

摘　　要：目的 :评价成人继发孔房间隔缺损 (ASD)封堵术后左心大小及功能的变化。方法 :全组患者 3 0例 ,年龄 15～5 6(2 9± 14 )岁。均于术前、术后 48h、术后 3月行超声心动图检查。结果 :ASD封堵术后 48h,左心室大小和左室功能与术前无明显差异。与术前、术后 48h比较 ,术后 3月左心室前后径和横径显著增加 (P<0 .0 1) ;左室每搏量 ,左室射血分数 ,左室短轴缩短率亦明显增加 (P<0 .0 1或 P<0 .0 5 )。但左心室舒张期末最大长径和左房长、横径均无显著变化 (P>0 .0 5 )。结论 :Amplatzer封堵器 ASD封堵术后 48h患者左心室已开始增大 ,左心功能有所增增加 ,但差异未达到显著水平。术后 3月左心室明显增大 ,左室功能显著改善。AIM: To evaluate the changes in the left cardiac size and cardiac function after transcatheter closure of adult secundum atrial septal defect (ASD) and during the follow up. METHODS: 30 adult patients aged from 15 to 56 years (29±14 years) with secundum ASD underwent echocardiographic examination before and 48 h after transcatheter closure and during 3 mo follow up. RESULTS: No significant difference was found between the left cardiac size and function 48 h after transcatheter closure of ASD and those before the operation. The anteroposterior diameter and transverse diameter of left ventricule increased significantly ( P <0.01) during 3 mo follow up compared with those before and 48 h after transcatheter closure of ASD,left ventricular stroke volume, left ventricular ejection fraction and shortening fraction also increased significantly ( P <0.01 or P <0.05), but left ventricular end diastolic maximum longitudinal diameter and the inner diameter of left end systolic atrium did not increase significantly( P >0.05). CONCLUSION: The left cardiac size and function did not increase significantly 48 h after the transcatheter closure of ASD, but increased significantly 3 mo after the transcatheter closure.
