中西医结合治疗结核菌素试验阳性患者临床疗效观察

摘　　要：目的探讨中西医结合治疗结核菌素试验阳性患者的方法及疗效。方法选取自2013年1月至2016年5月体检期间发现的痰结核菌素试验阳性患者106例,随机分为西医组与中西医结合组,每组各53例患者。西医组采用2HRZE/4HR抗结核方案治疗;中西医结合组在西医组治疗方案基础上,给予患者健脾润肺丸0.9~1.2 g,3次/d,持续治疗6个月。观察并比较两组患者治疗后的免疫功能指标、中医证候积分及临床疗效。结果治疗后,两组患者血清CD3+、CD4+、CD8+、CD4+/CD8+比较,差异均有统计学意义(P<0.05)。治疗后,两组患者消瘦、潮热、盗汗的中医证候积分比较,差异均有统计学意义(P<0.05)。治疗后3个月(66.0%比83.0%)及6个月(69.8%比88.7%),中西医结合组痰结核菌涂片阳性率均低于西医组,两组间比较,差异均有统计学意义(P<0.05)。结论中西医结合治疗结核菌素试验阳性的疗效确切,有助于提高免疫功能,改善中医临床证候。Objective To investigate the clinical effect of combination of traditional Chinese and Western medicine in treating smear positive pulmonary tuberculosis. Methods A retrospective study was performed on 106 cases of patients with smear positive pulmonary tuberculosis from January 2013 to May 2016. Patients were randomly divided into the Western medicine group and Chinese and Western medicine group,with 53 cases in each group. Patients in the Western medicine group were given 2 HRZE/4 HR anti tuberculosis therapy; the others in the Chinese and Western medicine group were given additional Jianpi Runfei pill 0. 9-1. 2 g,3 times per day,lasted for 6 months. The immunological function indexes,TCM syndrome integrals and clinical curative effects were observed and compared between the two groups. Results After treatment,the levels of CD3 ＋,CD4 ＋,CD8 ＋ and CD4 ＋/CD8 ＋ had statistically significant differences between the two groups（ P〈0. 05）; the TCM syndrome integrals of weight loss,hot flashes and night sweats had statistically significant differences between the two groups（ P〈0. 05）. At 3 months and 6 months after treatment,the sputum smear positive rate in the Chinese and Western medicine group（ 66. 0% versus 83. 0%） and（ 69. 8% versus 88. 7%） were lower than those in the Western medicine group,and the differences were statistically significant（ P〈0. 05）. Conclusion Combination of traditional Chinese medicine and Western medicine is effective in the treatment of smear positive pulmonary tuberculosis,which is helpful for improving immune function and clinical TCM symptoms.
