益气养阴摄血汤治疗肺结核咯血的临床研究

摘　　要：目的:观察益气养阴摄血汤治疗肺结核咯血的临床疗效。方法:选取152例肺结核咯血患者为研究对象,随机分为对照组(n=76)与研究组(n=76)。对照组采用垂体后叶素治疗,研究组在对照组基础上应用益气养阴摄血汤治疗。比较两组治疗前后临床症状、生存质量的变化情况及止血时间、住院时间、不良反应发生情况,并对两组临床疗效进行评价。结果:研究组临床疗效总有效率、不良反应发生率、止血时间、住院时间分别为93.42%(71/76)、3.95%(3/76)、(2.85±1.16)d、(12.22±2.38)d,均优于对照组[80.26%(61/76)、14.47%(11/76)、(3.88±1.07)d、(16.47±3.89)d],差异均有统计学意义(P<0.05);与治疗前比较,治疗后两组中医证候积分下降,而生存质量评分上升,且治疗后研究组中医证候积分、生存质量评分均优于对照组,差异均有统计学意义(P<0.05)。结论:益气养阴摄血汤治疗肺结核咯血患者,可显著提高临床疗效,缓解临床症状,减轻痛苦,缩短止血及住院时间,降低不良反应的发生,促进身体快速恢复,提高生存质量。Objective： This study aims to observe the clinical efficacy of Yiqi Yangyin Shexue decoction in the treatment of hemoptysis during pulmonary tuberculosis. Methods： A total of 152 patients with hemoptysis during pulmonary tuberculosis in our hospital were randomly divided into control group（n =76） and research group（n=76）. Control group was treated with hypophysin, research group took Yiqi Yangyin Shexue decoction on the basis of control group. The changes of clinical symptoms, survival quality and hemostatic time, hospital stays, occurrence of adverse reactions of two groups before and after treatment were compared. The clinical efficacy of two groups were evaluated. Results： The clinical total effective rate, incidence of adverse reactions,hemostatic time, hospital stays of research group were 93.42%（71/76）, 3.95%（3/76）,（2.85±1.16） d,（12.22±2.38） d respectively, which were better than control group [80.26%（61/76）, 14.47%（11/76）,（3.88±1.07） d,（16.47±3.89） d]（P〈0.05）; Compared with pre-treatment, the TCM symptom scores of two groups decreased after treatment, while survival quality scores increased, and the TCM symptom score and survival quality score of research group after treatment were better than control group（P〈0.05）. Conclusion： Yiqi Yangyin Shexue decoction for patients with hemoptysis during pulmonary tuberculosis can significantly improve clinical efficacy, relieve clinical symptom, reduce pain, shorten hemostatic time and hospital stays, decrease the occurrence of adverse reactions, promote rapid recovery and increase survival quality, which is worthy of further generalization.
