摘  要:目的探讨血浆D-二聚体水平在膝关节置换术后并发深静脉血栓(DVT)早期诊断中的意义。方法选取我院收治的77例膝关节置换术患者作为膝关节置换组,分别于术前及术后第1、3、7天进行血浆D-二聚体水平检测,并根据彩色多普勒超声检查结果将其分为DVT组(n=24)和非DVT组(n=53)。选取同期体检的健康者70例作为对照组。比较各组间的D-二聚体水平。结果术后1、3、7 d,两组膝关节置换术后患者的的血浆D-二聚体水平明显高于术前1 d,且DVT组明显高于非DVT组(P<0.05);DVT组患者术后1、3、7 d的D-二聚体水平明显高于对照组(P<0.05);非DVT组患者术后1、3、7 d的D-二聚体水平明显高于对照组(P<0.05)。结论血浆D-二聚体水平检测对膝关节置换术后并发深静脉血栓的早期诊断具有重要的临床意义。Objective To investigate the significance of plasma D-dimer level in early diagnosis of deep venous thrombosis(DVT) after knee arthroplasty. Methods A total of 77 patients undergoing knee arthroplasty surgery in our hospital were selected as the knee arthroplasty group, and all the patients received plasma D-dimer detection before operation and1, 3 and 7 days after operation. They were divided into DVT group(n=24) and non-DVT group(n=53) according to the results of color Doppler ultrasound. Other 70 physical examiners in the same period were enrolled as the control group. The plasma D-dimer levels were compared among the groups. Results At 1, 3 and 7 days after operation, the plasma D-dimer levels of patients undergoing knee arthroplasty surgery in the two groups were higher than those 1 day before operation, and those of the DVT group were higher than the non-DVT group(P<0.05). At 1, 3 and 7 days after operation, the plasma Ddimer levels of the DVT group were higher than those of the control group(P<0.05). At 1, 3 and 7 days after operation, the plasma D-dimer level of the non-DVT group was higher than that of the control group(P<0.05). Conclusion Plasma Ddimer level in early diagnosis of DVT after knee arthroplasty has vital clinical significance.
