2015年新疆麻疹病例诊断符合初步分析

摘　　要：目的了解新疆各级医疗机构麻疹报告病例诊断的准确性,为麻疹疫情的防控提供依据。方法分析2015年1月1日—12月31日新疆所有医疗机构报告的6 045例麻疹相关病例,计算病例的就诊及时性、诊断及时性和诊断符合率等指标。结果 2015年新疆麻疹病例诊断符合率为45.66%(2 746/6 014),诊断符合率实验室确诊病例为52.11%(2 615/5 014)、临床诊断病例为14.01%(131/935);病人从发病到就诊平均时间3 d,初诊到终诊平均时间5 d;445例重复报告病例中,56.85%为确诊病例。结论新疆各级医疗机构麻疹病例诊断符合符率不高,就诊和确诊时间间隔较长,病人存在重复和多头就医现象;开展强化麻疹疫苗接种的同时,应加强传染源的防控管理,做到早发现、早诊断、早治疗,有效地控制麻疹的传播与扩散。Objective To understand the diagnostic accuracy of measles reported by the medical institutions in Xinjiang,and provide evidences for measles control. Methods The timeliness of clinical visit and diagnosis, and the accurate rate of diagnosis were analyzed in 6 045 measles related cases reported by the medical institutions in Xinjiang from January 1 to December 31, 2015. Results The diagnostic accordance rate of measles cases, lab examination and clinical diagnosis was 45.66%（2 746/6 014）, 52.11%（2 615/5 014） and 14.01%（131/935） respectively in Xinjiang, 2015. The average duration was 3 days, from onset to clinical visit, and 5 days from the first visit to final diagnosis. Out of the 445 cases reported repeatedly, there were 56.85% of the cases with confirmed diagnosis. Conclusions The proportion of diagnostic accuracy of measles is not high in Xinjiang medical institutes. The duration from the first visit to final diagnosis is long. Some patients have to visit doctor repeatedly or visit multiple hospitals. The suggestion is to strengthen measles vaccination. Meanwhile,the management of infection sources should be reinforced for measles prevention and control. Early detection, diagnosis and treatment should be carried out for measles control effectively.
