综合护理干预对PICC置管患者导管维护依从性及并发症的影响

摘　　要：目的探讨综合护理干预对PICC置管患者导管维护依从性及并发症的影响。方法选择2013年1月至2016年10月期间解放军第303医院感染科收治的126例结核性脑膜炎患者,所有患者均使用20%甘露醇,均置入PICC导管,根据随机数表法分为观察组与对照组,每组63例。对照组予常规护理,观察组予综合护理干预,干预周期为3个月(包括出院后的随访)。干预后,比较两组患者导管维护依从率及并发症发生率。结果观察组患者的导管维护依从率为95.24%,明显高于对照组的84.13%,差异有统计学意义(P<0.05);观察组患者的PICC置管并发症发生率为4.76%,明显低于对照组的14.29%,差异有统计学意义(P<0.05)。结论综合护理干预可明显提高PICC置管患者导管维护依从性,降低并发症发生率。Objective To investigate the influence of comprehensive nursing intervention on catheter mainte- nance compliance and complications in patients with peripherally inserted central catheter （PICC）. Methods A total of 126 cases of tuberculous meningitis patients in Department of Infectious Diseases, the 303rd Hospital of PLA from Janu- ary 2013 to October 2016 were selected. All patients were treated with 20% mannitol, and PICC cathets were implanted. According to the random number table, the patients were divided into the observation group and control group, with 63 patients in each group. The control group received routine nursing care, and the observation group were given com- prehensive nursing intervention, with the intervention period for 3 months （including fo｜low-up after discharge）. The compliance rate of catheter maintenance and complication were compared between the two groups after intervention. Results The catheter maintenance compliance rate was 95.24% in observation group, which was significantly higher than 84.13% in the control group （P〈0.05）. The PICC catheter complication rate of observation group was 4.76%, which was significantly lower than 14.29% of the control group （P〈O.05）. Conclusion Comprehensive nursing intervention can significantly improve the compliance of catheter maintenance in patients with PICC catheter, and reduce the inci- dence of complications.
