摘　　要：在介绍延续护理的概念与优点的基础上,重点阐述了国外较为成熟的并且能够应用于老年COPD患者的六大延续护理模式,提出我国应用延续护理所面临的挑战,希望为进一步开展我国的延续护理模式构建提供参考.Besides introducing the concept and advantages of extended nursing care, this paper focuses on six extended nursing care models which are mature and can be applied to elderly chronic obstructive pulmonary disease （COPD） patients in foreign counties. It also puts forward the challenges faced by the application of extended care in China, hoping to provide references for further development of extended nursing care model in China.
