高频彩色多普勒超声在睾丸扭转诊断中的应用价值

摘　　要：目的探讨高频彩色多普勒超声在睾丸扭转诊断中的应用价值。方法另选取吉安市遂川县人民医院2016年1月—2017年1月收治的疑似睾丸扭转患者60例为研究组,选取本院同期健康体检者50例为对照组,分别进行高频彩色多普勒超声检查,并经手术病理诊断确诊。比较两组受试者睾丸体积、纵切面睾丸上径与前后比值(L/S),并比较研究组不同睾丸扭转发生时间、程度的失睾率。结果研究组中手术病理诊断睾丸扭转57例,高频彩色多普勒超声诊断准确率为95%。两组受试者睾丸体积、L/S比较,差异有统计学意义(P<0.05)。研究组发病时间<24 h失睾率低于>24 h(P<0.05)。<180°失睾率低于180°~360°(P<0.05),扭转程度180°~360°与>360°失睾率比较,差异无统计学意义(P>0.05)。结论尽早诊断和治疗睾丸扭转可降低失睾率,高频彩色多普勒超声诊断睾丸扭转能够清晰反映其结构和血流情况,以为患者治疗提供可靠依据。Objective To study the application value of high frequency color doppler ultrasound in the diagnosis of testicular torsion.Methods A total of 60 cases of suspected testicular torsion patients were selected from January 2016 to January 2017 in Suichuan County People＇s Hospital as study group,at the same time,50 cases of health researchers were selected as control group,all patients were examined by high frequency color doppler ultrasound,and confirmed by surgery pathology.The testicular volume,L/S were compared between the two groups,and the testosterone loss rate of different testicular torsion time and degrees were compared in study group.Results In study group,57 cases of patients were diagnosed testicular torsion,the diagnostic rate was 95%.There were statistically significant differences of testicular volume,L/S between the two groups（P〈0.05）.In study group,testosterone loss rate in onset time 24 h was lower than onset time 24 h（P〈0.05）,testosterone loss rate of testicular torsion 180° was lower than 180° ～ 360°（P〈0.05）,no statistically significant difference of testosterone loss rate was found between testicular torsion 180° ～ 360° and 360°（P〈0.05）.Conclusion Timely diagnosis and treatment can reduce testosterone loss rate,high frequency color doppler ultrasound can visual display the testis morphology and blood supply situation to provide a reliable basis for early diagnosis and treatment of patients.
