综合干预对手深度烧伤功能康复的影响

摘　　要：目的探讨手部深度烧伤功能康复的有效对策。方法选择2013年1月~2015年12月收治的142例手深度烧伤住院患者作为干预组,2011年1月~2012年12月的140例为对照组,干预组采取综合康复干预措施,对照组采取常规康复,分析两组患者干预后3、6个月时手的外观情况以及出院当天及1、3、6个月时的手功能恢复情况。结果采用温哥华瘢痕量表(Vancouver scar scale,VSS)评价,结果显示,两组患者干预后3、6个月手部瘢痕(外观)情况具有明显差异(P<0.05),提示干预组优于对照组;上肢功能评价表(DASH)评价结果显示,两组患者出院当天及1、3、6个月时上肢功能康复情况差异有统计学意义(P<0.05),提示干预组优于对照组。结论实施综合干预措施能有效促进手的功能康复和外观的改善,让手深度烧伤患者早日回归家庭,回归社会。Objective To explore the effective strategy of hand rehabilitation after deep burns. Methods 142 patients with deep burned hands were randomly included in the intervention group from Jan 2013 to Dec 2015, which received comprehensive nursing interventions; and 140 patients with deep burned hands were included in the control group before January 2013, which accepted traditional routing nursing methods. The appearance of the hands in the two groups was analyzed after 3 and 6 months. The rehabilitation of the hand function in the two groups was analyzed at discharge and after 1, 3, and 6 months. Results The appearance of hand scars after 3 and 6 months were significantly different （P＜0.05），which the intervention group was better than the control group by Vancouver Scar Scale. The rehabilitation of the hand function was statistically significantly different （P＜0.05） at discharge, after 1, 3, and 6 months，which the intervention group was better than the control group by DASH. Conclusion The comprehensive nursing intervention can effectively promote the rehabilitation of the appearance and function, leading to the patients with deep burned hands returning to the family and the society early.
