摘  要:目的分析DTI、BOLD-fMRI在视束萎缩中的影像学表现,进行量化分析,评价P-VEP、DTI与BOLDf MRI在视束萎缩中的功能应用。方法对11例视束萎缩患者及15例正常对照组行3.0T MRI常规T_1WI、T_2WI、DTI及BOLD-fMRI、P-VEP及视野检查,将病变组检查结果与正常对照组进行对比分析。结果 (1)病变组与正常对照组比较,T_1WI、T_2WI示患侧视束均变细,信号未见明显异常;视野均呈同向偏盲;(2)病变组视束FA值、λ∥值和视皮层激活体素数较对照组明显降低,ADC值和λ_⊥值较健侧明显升高,二组间差异均存在统计学意义(P<0.001);病变组P-VEP较正常组P_(100)振幅降低,潜伏期延长,二组间差异均存在统计学意义(P<0.001)。结论 DTI可定量反映视束萎缩的神经纤维的细微病理改变,BOLD-fMRI能无创反映视皮层功能区部位及量的变化,P-VEP能反映视觉冲动的传导速度,因此P-VEP、DTI联合fMRI-BOLD可成为评价视束形态及功能改变的客观指标,为视束萎缩的临床诊断、预后评估及深入研究提供新的方法。Objective To analyze the imaging manifestations of DTI,BOLD-MRI in optic tract atrophy,and evaluate the value of P-VEP,DTI combined with BOLD-fMRI in optic tract atrophy. Methods Totally 11 patients with optic tract atrophy and 15 age-matched healthy controls were enrolled in this study. Measures of 3. 0T MRI( convention T1WI,T2WI),DTI,BOLD-fMRI,P-VEP and visual field were performed on all subjects. Corresponding parameters of DTI,BOLD-fMRI,P-VEP and visual field were obtained for quantitative analysis,respectively. Results(1)On conventional MRI,the size of optic tract in patient group was fine but with normal signal and all patients were suffered with homonymous hemianopia.(2)Compared with controls,the optic tract of patients showed significantly lower FA,λ//,P_(100) and active volumes of visual cortex,and significantly higher ADC and λ_⊥( P 〈0. 001). Conclusion DTI can provide in vivo information about pathology of the optic tract atrophy and BOLD-fMRI can detect changes in the visual cortex. Moreover,P-VEP can reveal the transmission speed of visual impulsion. Thus,P-VEP,DTI combined with BOLD-fMRI can be used to assess the alterations in optic tract shape and function,and may be a new method for clinical diagnosis,prognosis,and further research in optic tract atrophy.
