摘　　要：目的探讨炎症型肝细胞腺瘤（HA-I）MRI的表现特征。方法回顾性分析经病理及免疫组织化学证实的9例HA-I患者13个病灶的MRI表现,评价MRI各序列信号特征及动态增强各期强化特点。结果 T2WI表现为中等高信号及明显高信号的病灶分别为4个及9个,DWI表现为中等高信号及明显高信号的病灶分别为5个和8个;ADC图表现为等信号12个;动态增强动脉期明显强化11个;且持续强化12个。2个病灶内有少量脂肪,2个合并出血、坏死及囊变,6个周围有异常信号环。结论 MRI检查可显示HA-I的影像特征,能反映病灶内脂肪、出血坏死等病理特点。Objective To evaluate the diagnostic performance of the imaging features of the inflammatory hepatocellular adenoma（ HA-I） with MR. Methods MR imaging findings of 9 patients with 13 HA-Is proven histopathologically were retrospectively analyzed. The signal intensity in precontrast sequences,the dynamic enhancement pattern and intensity in postcontrast sequences,the presence of steatosis in the nontumoral liver and fat deposits,hemorrhage,cystic necrosis,rim formation in the lesion were assessed and compared with the pathologic analysis as the standard of reference. Results Moderately or markedly hyperintense signal on T2 WI was observed in 4 of 13 HA-Is and 9 in 13 HA-Is respectively. Moderately or markedly hyperintense on DWI were in5 of 13 HA-Is and 8 in 13 HA-Is respectively,isointense signal on ADC was observed in 12 of 13 HA-Is. Intense arterial enhancement were in 11 of 13,and persisted enhancement in venous and delayed phases in 12 of 13 HA-Is. 2 lesions presented with fat deposits,2 lesions presented with hemorrhage and cystic necrosis. Rim formation could be found in 6 lesions. Conclusion The MR imaging features of moderately or markedly hyperintense signal on T2 WI and DWI associated with persistent enhancement in venous and delayed phases may suggest the subtype of HA-I in the differential diagnosis.
