以反晕征为主要CT表现的肺结核三例并文献复习

摘　　要：目的分析以反晕征为主要CT表现的肺结核患者的影像表现特点,以提高对这类不典型肺结核的诊断准确率。方法回顾性分析3例确诊的以反晕征为主要CT表现的肺结核患者的临床和影像资料,并复习相关文献,总结这类疾病的影像学特点、病理基础及鉴别诊断。结果 3例患者均为免疫正常人群,无明显结核中毒症状,肺内出现反晕征且合并有腺泡结节和树芽征,反晕征的壁及内部的磨玻璃区均可见结节影。结论肺内出现反晕征的肺结核患者影像学表现具有特征性,结合临床资料,有助于及时诊断和治疗。Objective To analyze the CT manifestation of pulmonary tuberculosis（ PTB） with reversed halo sign in order to improve diagnostic accuracy of this atypical PTB. Methods The clinical and CT data of three cases of confirmed PTB were retrospectively analyzed and related literature were reviewed,to summarize the CT features,pathological basis and differential diagnosis of this disease. Results 3 patients with normal immune system had no obvious TB-related symptoms,exhibited the reversed halo sign on chest CT with airspace nodules and branching linear and nodular opacities（ tree-inbud sign）,the reversed halo sign presented with nodular walls and nodules inside the ground-glass opacity. Conclusion The CT manifestations of pulmonary tuberculosis with reversed halo sign are characteristic,and it will be helpful for the diagnosis and treatment by combining imaging and clinic data.
